Objective:To study the expression of OPN(Osteopontin)in bladder urothelial carcinoma and normal bladder mucosa,explore the relationship between its expression and clinicopathological features,and analyze the clinical significance of OPN in bladder urothelial carcinoma.Methods:Immunohistochemical method was used to detect the expression of OPN in paraffin sections of 45 cases of bladder urothelial carcinoma tissues and 20 cases of normal bladder tissues.Combined with clinical data analysis,the statistical relationship between OPN expression and clinical stage,pathological grade and lymph node metastasis was analyzed.Then,the expression of OPN in 20 pairs of fresh bladder urothelial carcinoma tissues and adjacent tissues was quantitatively analyzed by Western blot and qRT-PCR to explore the relationship between OPN expression and clinical data such as age,sex,clinical stage,pathological grade of tumor,lymph node metastasis or not.Results:1.In the immunohistochemical results,OPN protein was located in cytoplasm,which was brown or brown.The positive expression rate of OPN in urothelial carcinoma(76%)was significantly higher than that in normal urothelial tissue(15%),and the difference was statistically significant(P<0.05).The positive rate of OPN protein expression was 50%in patients with Ta+T1 stage urothelial carcinoma and 82.4%in patients with T2-T4 stage urothelial carcinoma,and the difference between the two groups was statistically significant(P<0.05).The positive rate of OPN protein in patients with pathological grade Ⅰ and Ⅱ was 52.8%,and the positive rate of OPN protein expression in patients with grade Ⅲwas as high as 100%,and the difference between them was statistically significant(P<0.05).In patients with bladder urothelial carcinoma with lymph node metastasis,the positive expression rate of OPN protein reached 90%,while in patients without lymph node metastasis,the positive expression rate of OPN was only 54.3%,and the difference was statistically significant(P<0.05).The expression in bladder urothelial carcinoma had no statistical significance in sex and age(P>0.05).2.Western blot results showed that the expression level of OPN protein in bladder urothelial carcinoma was 1.56±0.16,while that in normal urothelium was 0.41±0.09,and the expression level in bladder cancer tissue was higher than that in normal bladder mucosa,and the difference was statistically significant(P<0.05).3.qRT-PCR results showed that the relative expression of SPP1 in bladder urothelial carcinoma was significantly higher than that in normal bladder tissue,and the difference was statistically significant(P<0.05).The expression level of SPP1(The gene that encodes OPN)was not related to age distribution,sex and lymph node metastasis,and the difference was not statistically significant(P>0.05).The expression level of SPP1 is related to clinical stage(muscle layer is invaded or not).The relative expression levels of SPP1 mRNA in patients with MIBC and patients with NMIBC are 26.85±1.13 and 20.23 ± 1.52,respectively.The expression level of SPP1 in patients with MIBC is significantly higher than that in patients with NMIBC.According to the pathological classification,the relative expression of mRNA in grade I and II was lower than that in grade Ⅲ,which were 19.85±0.85 and 26.54±1.30.The expression of SPP1 in grade Ⅲ(high grade papillary urothelial carcinoma)was higher than that in grade Ⅰ and Ⅱ(low grade papillary urothelial carcinoma and low grade papillary urothelial carcinoma).The difference between the two groups was statistically significant(P<0.05).Conclusion:The expression of OPN protein in bladder urothelial carcinoma is significantly higher than that in normal urothelium.With the increase of clinical stage and pathological grade,the expression level of OPN protein increases,and OPN may be involved in the occurrence and development of bladder urothelial carcinoma.OPN has certain significance for early diagnosis of bladder cancer,monitoring indicators of postoperative reexamination and prognosis evaluation.It is expected to be a tumor marker of bladder cancer.
