Objective:To study the long-term drug use patterns and addiction of patients taking Benzodiazepines(BZDs)in the psychiatric outpatient department of the Municipal Psychiatric Center,analyze the general demographic,pharmacological and clinical characteristics of the patients,and study the long-term use of benzodiazepines(BZDs).Addiction rate and related influencing factors,in order to provide more abundant clinical data for the long-term application of BZDs and addiction situation,so that psychiatrists can use BZDs reasonably and effectively in the face of patients with different diseases and conditions,and take effective measures.The means to avoid risks and addictions in a timely manner makes the clinical application of BZDs more reasonable and safe.Methods:The experimental group selected patients with long-term use(≥6 months)of benzodiazepines who visited the psychiatric outpatient department of Jining Psychiatric Center from July to December 2020,and used sedative and hypnotic drugs in the "DSM5 Diagnostic and Statistical Manual of Mental Disorders" Relying on the diagnostic criteria,a self-made questionnaire was used to record general demographic characteristics,clinical characteristics of psychiatry,drug use and addiction.Descriptive statistical methods were used to report the characteristics of long-term use of BZDs in psychiatric outpatient clinics.Logistic regression analysis was further used.to demonstrate factors associated with chronic use,misuse,and dependence.SPSS 23.0 was used for data analysis.2.Results:1.The mean duration of benzodiazepine use in the surveyed patients was 8.13 years,the median was 5.5 years,and the interquartile range was 8 years.The median number of outpatient visits was 19.5 with an interquartile range of 20;the median number of hospital admissions was 0.58 with an interquartile range of 2,and the onset was mainly chronic;the median disease duration distribution was 8.5 years,with an interquartile range of 10 years;patients received a mean daily dose of 3.6 diazepam 5 mg equivalents for a median duration of 5.5 years,with an interquartile range of 8 years.3.2.The addiction rate of all enrolled patients using BZDs was 20.1%;the addiction rate of females was higher than that of males;compared with non-addicted patients,addicted patients had higher physical illnesses,fewer hospitalizations,longer duration of disease,and longer medication time Prolonged;Addiction rates were significantly higher in patients taking 2 BZDs,continuous BZD use,previous use of other BZDs,and other sleep aids than those without these characteristics.4.3.The results of binary logistic regression analysis showed that the risk factors of BZDs addiction were female,age>60 years old,currently retired or unemployed,combined use of two BZDs,continuous use without discontinuation,combined use of olanzapine,combined with other sleep aids drugs And the diazepam equivalent,disease duration and addiction rate of BZDs were positively correlated.Conclusion:1.Long-term users of benzodiazepines are mainly middle-aged and elderly women,married,retired or unemployed,those with low education level(below junior high school),and no history of smoking and alcohol;the disease diagnosis is mainly insomnia and neurosis;Most of them have chronic disease course,negative family history,no medical disease and medication history;most of the medication is for insomnia or anxiety;alprazolam and clonazepam are the most frequently used drugs for long-term use;5.Risk factor analysis suggests that in clinical work,BZDs should be used with caution for elderly women and people living alone without work;during the medication process,try to use a single medication and avoid the combination of multiple BZDs;avoid long-term continuous use,if the patient’s symptoms are relieved,it can be reduced.Dosage or discontinuation;try not to combine with other antipsychotics such as olanzapine and other sleep aids.
