Objective:To understand the breast health status and influencing factors of rural women in Jinan,and to explore the impact of environmental pollution on female breast nodule diseases in rural areas,to provide data support for the prevention of breast diseases.Methods:From July 2019 to September 2019,women who participated in the screening of "two cancers"(breast cancer and cervical cancer)in rural areas of Jinan,Shandong Province,were examined by high frequency ultrasound,and the relevant medical history was inquired,the results were recorded on the spot and archived.screen out people with breast diseases.Taking breast nodular disease(BI-RADS Ⅲ ~ V)as a positive index,the living environment of the studied women was investigated in the field,and the exposure to environmental pollution was evaluated.Combined with ultrasound data and environmental pollution related data,statistical analysis was carried out to explore the relationship between environmental pollution and breast nodular disease.Results:1.Among the 4082 rural women screened for "two cancers" in this study,1272 were positive by breast ultrasound,accounting for 31.2%.The main types of diseases in ultrasound results were breast hyperplasia(19.1%),breast space occupying lesions(9.6%),non-lactation mammary duct dilatation(1%)and so on.There were significant differences in the incidence of breast diseases among different age groups(χ2 = 528.7,P < 0.05).Among them,the incidence of breast diseases in the 40 ~ 50 age group was higher than that in other age groups.There was a significant difference in the incidence of breast disease among the groups with different parity(χ2 = 61.9,P < 0.05).The incidence of breast disease was higher in the group with 3 parity.Multivariate logistic regression showed that the risk of breast disease among rural women in this area increased with age,among which the risk of breast disease was the highest in the 40~50 group(OR = 11.58,95%C I= 8.38~16.01),followed by the 50~60 group(OR = 8.10,95%CI = 6.14~10.73),and the risk of breast disease in the 60~65 group was lower than that in the former two groups(OR = 2.14,95%CI = 1.62~2.84).The factors of parity order were not statistically significant in multivariate logistic regression analysis(P > 0.05).2.Among the 4082 rural women selected in this study,the detection rate of breast nodules(BI-RADS grade Ⅲ ~ Ⅴ)by breast ultrasound was 9.6%(390/ 4082).There were significant differences in the detection rate of breast nodules between the two groups(χ2 = 92.27,11.60,23.92,8.60,19.88 and 6.34,P < 0.05).There were significant differences in the detection rate of breast nodules between the two groups(χ2= 92.27,11.60,23.92,8.60,19.88 and 6.34,P < 0.05).There were significant differences in the detection rate of breast nodules between groups(χ2 = 92.27,11.60,23.92,8.60,19.88 and6.34,P < 0.05).Multivariate logistic regression analysis showed that the risk factors,OR(95%CI)of breast nodule disease were 4.68(2.97 ~ 7.38),1.36(1.02 ~ 1.80)and 1.35(1.04 ~ 1.75),respectively.Conclusion:1.The incidence of breast disease is high in rural areas of Jinan.The extensive screening program for "two cancers" of rural women is helpful to detect breast diseases as early as possible and improve the health level of rural women.2.The detection rate of breast nodule disease is high in women in rural areas of Jinan,and environmental pollution is an important risk factor for breast nodule disease.
