| Objective1.To understand the information needs of breast cancer patients in the preoperative waiting room.2.To explore the effects of information support combined with relaxation training on anxiety and vital signs of breast cancer patients,occurrence and duration of restlessness in anesthesia recovery room,and to understand patients’satisfaction with operating room nursing services and subjective feelings about intervention measures to provide reference basis for stabilizing the psychological state of breast cancer patients in preoperative waiting room.Methods1.Semi-structured interviews:Using the maximum difference sampling method in objective sampling,patients who were scheduled to undergo modified radical mastectomy before operation were selected for semi-structured interviews,and the interview data were analyzed by content analysis method to explore the information needs of breast cancer patients in the preoperative waiting room.2.Quasi-experimental study:On the basis of semi-structural interviews and expert meeting,form an information support video capture scheme,on the basis of the literature review and expert meeting form a relaxation training intervention plan,After the intervention process was modified and improved through pre-test,the final scheme was applied to the patients who planned to undergo modified radical mastectomy,and its intervention effect was discussed.A total of 188 patients who were admitted to a third-class hospital in Qingdao from January 2020 to December 2020 and met the inclusion and exclusion criteria were selected as the study subjects.The patients were randomly divided into control group,information support group,relaxation training group and combination group.The control group received routine preoperative visit and nursing care in the preoperative waiting room,the information support group provided information support by video on the basis of the control group,the relaxation training group received progressive muscle relaxation training in the preoperative waiting room on the basis of the control group and the combination group carried out the intervention measures of the information support group and the relaxation training group at the same time on the basis of the control group.Anxiety score,vital signs were compared on the day before surgery,entering and leaving the preoperative waiting room.The incidence of agitation in anesthesia recovery room,the duration of stay in anesthesia recovery room and the Patients’satisfaction with operating room nursing services were compared among the four groups,understand the subjective feelings of patients in the intervention groups.Microsoft Excel 2016 software was used for data input,IBM SPSS Statistics 26.0 software was used for data processing and single factor analysis of variance,repeated measurement analysis of variance,nonparametric test,X~2 test,Fisher’s exact probability test were used for statistical analysis of the data.Results1.Semi-structured interviews:This study conducted semi-structured interviews with 16 breast cancer patients,and extracted five topics about the information needs of such patients in the preoperative waiting area:"disease and related treatment needs","operating room environmental needs","operation process needs","psychosocial support needs"and"multi-form information provision".2.Quasi-experimental study:(1)Comparison of general data of four groups of patients.There was no significant difference in age,occupation,education level,number of operation tables,ASA grade,operation site and waiting time before operation among the four groups(P>0.05).(2)Repeated measurement ANOVA results showed that the intergroup,time and interaction effects of anxiety,blood pressure and heart rate at different time points in the four groups were statistically significant(P<0.05).There were no statistically significant differences in anxiety,blood pressure and heart rate on the preoperative day(P>0.05),while there were statistically significant differences in anxiety,blood pressure and heart rate when entering and leaving the preoperative waiting room(P<0.05).Further pairwise comparison showed that:When entering the preoperative waiting room,the anxiety score,blood pressure and heart rate of the control group and the relaxation training group were higher than those of the information support group and the combination group(P<0.05),There was no statistical significance between the control group and the relaxation training group(P>0.05),and there was no statistical significance between the information support group and the combination group(P>0.05).When leaving the preoperative waiting room,the anxiety score,heart rate and blood pressure of the control group were higher than those of the other three groups(P<0.05),and the anxiety score,heart rate and blood pressure of the combination group were lower than those of the other three groups(P<0.05).There was no significant difference between the information support group and the relaxation training group(P>0.05).(3)There was significant difference in the length of stay in the anesthetic recovery room among the four groups(P<0.05).Further pairwise comparison showed that the length of stay in the anesthetic recovery room in the combination group was significantly shorter than that in the other three groups(P<0.05).There was no significant difference among control group,information support group and relaxation training group(P>0.05).(4)There was no significant difference in the incidence of restlessness in anesthesia recovery room among the four groups(P>0.05).(5)There was significant difference in the satisfaction score of nursing service in operating room among the four groups(P<0.05).Further pairwise comparison showed that the satisfaction score of information support group,relaxation training group and combination group was higher than that of control group.The satisfaction score of the combination group was higher than that of the information support group and the relaxation training group,and the difference was statistically significant(P<0.05).There was no significant difference in satisfaction score between information support group and relaxation training group(P>0.05).(6)There was no significant difference in any items of subjective feelings of intervention in the intervention group(P>0.05),but there was significant difference in total score(P<0.05).Further pairwise comparison showed that the total score of the combination group was significantly higher than that of the information support group and the relaxation training group(P<0.05),but there was no significant difference between the information support group and the relaxation training group(P>0.05).Conclusions(1)The information needs of breast cancer patients in the preoperative waiting room mainly include disease and related treatment needs,operating room environment needs,operation process needs,psychosocial support needs,and various forms of information provision.(2)Information support combined with relaxation training can alleviate the anxiety status of breast cancer patients in the preoperative waiting room,stabilize their vital signs,shorten the length of stay in the anesthesia recovery room,and improve patient’s satisfaction,but has no effect on the occurrence of restlessness in the anesthesia recovery room.Patients have good subjective feelings about information support and relaxation training,which has the value of clinical promotion and application. |
