Observation On The Curative Effect Of Self-made Tongshu Powder Fumigation On Early Rehabilitation After Rotator Cuff Repair

Objective:To observe the effect of self-made Tongshusan fumigation on postoperative pain,range of motion and function of the shoulder with rotator cuff repair.By using self-made Tongshusan fumigation 6 weeks after rotator cuff repair,so as to provide theoretical basis for the application of self-made Tongshusan fumigation after rotator cuff repair to promote blood circulation,remove blood stasis,detumescence,relieve pain and promote joint function.And further guide the application of traditional Chinese medicine in early rehabilitation after rotator cuff repair.At the same time,give full play to the characteristics of traditional Chinese medicine nursing treatment.Methods:In this study,60 patients with rotator cuff repair in the Department of Sports Medicine of the First Affiliated Hospital of Guangzhou University of Chinese Medicine from September 2020 to January 2021 were selected and randomly divided into two groups,30 cases in each group.The control group was given routine orthopedic nursing(psychological nursing,diet nursing,scientific shoulder function exercise and so on).The experimental group was given routine orthopedic nursing and Tongshusan fumigation.The shoulder pain,range of motion,activities of daily living,range of motion and skin were observed and compared.SPSS 23.0 statistical software was used for data analysis.The measurement data were expressed as sx ±,and the two samples were compared by t-test;the count data were expressed as percentage(%),and compared by chi square test;one way ANOVA was used if multiple comparisons were made.P<0.05 was statistically significant.Results:All the 60 patients were followed up for 7 days.After statistical analysis,there was no significant difference in VAS pain score(at rest and in motion),shoulder range of motion(flexion,extension,abduction,adduction,lateral rotation),Constant-Murley Score,Pittsburgh sleep quality index and activities of daily living(ADL)score between the two groups before treatment(P>0.05)。After 7 days of self-made Tongshusan powder fumigation treatment,the results showed that:Comparative analysis of VAS pain score:on the 5th day of treatment,the VAS pain score of the experimental group at rest was 0.83±0.65,lower than 0.93±0.58 of the control group,the difference was statistically significant(P<0.001);the 7th day of treatment,the VAS pain score of the experimental group at rest was 0.43±0.51,lower than 0.53±0.51 of the control group,the difference was statistically significant(P<0.001);the VAS pain score at activity was 2.23±0.73,higher than 1.90±0.55 of the control group,but the difference was not statistically significant(P=0.07>0.05).Comparison and analysis of shoulder flexion range of motion:on the 7th day of treatment,the experimental group’s shoulder flexion range of motion was 166.83±11.71 degrees,which was better than 164.50±9.86 degrees in the control group,and the difference was statistically significant(P<0.001).Comparative analysis of shoulder joint extension range of motion:on the 7th day of treatment,the shoulder joint extension range of motion of the experimental group was34.17±10.67° and that of the control group was 36.83±6.50° with no significant difference(P=0.23>0.05).Comparative analysis of shoulder abduction range of motion:on the 5th day of treatment,the abduction range of motion of the experimental group was 143.17±19.85 degrees,which was better than 140.00±14.38 degrees of the control group(P<0.001);on the7 th day of treatment,the abduction range of motion of the experimental group was162.33±11.43 degrees,which was better than 160.67±7.40 degrees of the control group(P<0.001).Comparative analysis of shoulder adduction range of motion:on the 7th day of treatment,the adduction range of motion of the experimental group was 45.17±11.48° and that of the control group was 45.50±11.32 with no significant difference(P=0.89>0.05).Comparative analysis of shoulder lateral external rotation range of motion:on the 7th day of treatment,the shoulder lateral external rotation range of motion of the experimental group was 34.50±9.86° and that of the control group was 37.00±8.96° with no significant difference(P=0.72>0.05).Comparison and analysis of Constant-Murley Score:on the 7th day of treatment,the Constant-Murley Score of the experimental group was 61.03±3.60,which was better than58.23±3.35 of the control group,and the difference was statistically significant(P=0.01<0.05).Activity of daily living(ADL)score:on the 7th day of treatment,the ADL score of the experimental group was 90.00±0.00,which was better than 89.83±0.91 of the control group,and the difference was statistically significant(P=0.01<0.05).Conclusion:Under the guidance of traditional Chinese medicine theory,the self-made Tongshusan fumigation therapy was introduced into the rehabilitation of arthroscopic rotator cuff repair.Through early postoperative follow-up,it was found that compared with conventional rehabilitation therapy,the self-made Tongshusan fumigation combined with conventional rehabilitation therapy can better relieve shoulder pain,improve the flexion and abduction function of shoulder joint,and improve the shoulder joint function score.Although there was no significant improvement in shoulder joint extension,adduction and lateral external rotation compared with the control group,there were no adverse reactions such as scald and skin allergy after treatment.Therefore,the treatment is safe and effective,which is worthy of clinical promotion.
