Objective:To retrospectively analyze the difference of fecal tumor M2-PK concentration between colorectal adenoma patients and normal controls, who were chosed from the target population for CRC screening in the area of Zhejiang Jiashan, evaluate the clinical diagnostic value of this test for colorectal adenoma, and further discuss the value of this test for CRC screening within this area.Methods:Get fecal specimens from the target population before colonoscopy, and chose people with colorectal adenomas and randomly select normal controls without colorectal adenomas,both confirmed by colonoscopy, detect the fecal tumor M2-PK concentration using commercial kits. Results:The median value is3.8U/ml for case group, and3.6U/ml for control group, P=0.428>0.05, no significance; draw ROC curve to determine the best cut-off value as4.1U/ml, the corresponding sensitivity and specificity are40.5%and63.9%, respectively,χ2=0.151, P=0.697>0.05, no significance; AUC=0.512(95%CI:0.378~0.646), the diagnostic efficiency of fecal tumor M2-PK test for colorectal adenoma is low;advanced and non-advanced adenomas subgroups has no difference of statistical significance(P=0.319>0.05);fecal Tu M2-PK concentration has no correlation with gender (P=0.338>0.05) and age (P=0.112>0.05).Conclusion:This study gains a sensitivity slightly higher than previous similar study, but the specificity is low, so if add this test to screening programme, there will be more unnecessary colonoscopy, increasing screening cost and risk;the fecal tumor M2-PK concentration between case group and control group is of no significance, so under conventional sampling method, the diagnostic value of this test for colorectal adenoma is not so ideal,thus its role in the CRC screening within this area is limited and needed further study.
