| Objective To analyze the related risk factors of intracranial aneurysm rupture.Methods The clinical data and DSA imaging data of 103 patients(a total of 125aneurysms)diagnosed with intracranial aneurysm in the Department of Neurosurgery,Ningxia Hui Autonomous Region People’s Hospital affiliated to Ningxia Medical University from January 2016 to August 2020 were analyzed retrospectively.Among the 103 patients(a total of 125 aneurysms),there were 59 ruptured aneurysms and 44 patients with 66 unruptured aneurysms.General information included age,sex,hypertension,diabetes,heart disease,elevated blood lipids,reduced blood lipids,uric acid abnormality,smoking,drinking,receive antiplatelet drugs and family history of intracranial aneurysm.DSA imaging data include:aneurysm morphology,location,ascus,bifurcation,incidence Angle,multiple.One-dimensional parameters: width(W),diameter(D),height(H),neck width(N),intracranial aneurysm carrier artery diameter(PV);two-dimensional parameters: ARH、ARD、DWR、HWR、SRH、SRD.The aneurysms in the ruptured group and the unruptured group were compared.First,the single factor analysis was carried out on these factors.The risk factors were screened out with P<0.05 as statistically significant,The values of the detection parameters obtained by DSA were analyzed by ROC curve.Further multivariate Logistic regression analysis was performed to identify the independent risk factors for intracranial aneurysm rupture,and P < 0.05 was considered statistically significant.Results Single factor analysis results shows that smoking,high blood pressure,ascus,IA position,shape(irregular),incident Angle,uric acid increased,receive antiplatelet drugs,D,H,SRD,SRH,HWR,DWR,ARD,ARH is closely related to the rupture of intracranial aneurysm.The ROC curve shows that the area under the curve of ARD is the largest,and the AUC value is 0.743.The AUC values of D,H,,ARH,DWR,HWR,SRH and SRD were0.701,0.661,0.694,0.701,0.643,0.675 and 0.697,respectively.All had statistical significance(P<0.01).Logistic regression analysis results of influencing factors for intracranial aneurysm rupture showed that Chronic heavy smoking,merger of ascus,blood flow inflow angle≥90 ° and ARD is correlated with intracranial aneurysm ruptured hemorrhage.Conclusion Chronic heavy smoking,merger of ascus,blood flow inflow angle≥90°and ARD>1.499 is independent risk factors for rupture of intracranial aneurysm. |
