Objective To explore the relationship between the characteristic geometry and the risk of rupture in cerebral saccular aneurysm Methods The clinical features of 147 cerebral saccular aneurysm patients with complete materials of cerebral angiography,115with ruptured aneurysm and 32 with unruptured aneurysm, were analyzed retrospectively. Several geometrical parameters of the aneurysms were calculated and the relation of depth, neck width, diameter of parent artery,angle of inclination of aneurysms and aspect ratio(AR),size ratio(SR)and neck parentartery ratio(NPR)of aneurysms to the risk of rupture were analyzed. Results The mean depth, neck width, diameter of parent artery,angle of inclination of aneurysms and aspect ratio(AR),size ratio(SR)and neck parentartery ratio(NPR) of the patients with ruptured and unruptured aneurysms were(5.54±2.44)mm& (4.39±3.13)mm,(2.20±0.85) mm&(3.05±2.67)mm, (1.84±0.71)&(2.34±0.98) mm, (110±4)°&(102±3)°,(2.64±0.94) &(1.57±0.68)mm,(3.25±1.52)mm&(1.94±1.03)mm, (1.27±0.51)&(1.34±0.66)mm.Four of the parameters(depth, diameter of parent artery, AR,SR) were significantly statistical different between the ruptured and unruptured group. Respectively, all P<0. 05.Conclusion Both AR and SR are not corelated with the location of aneurysms and reflect the geometrical and hemodynamic features of cerebral saccular aneurysms well. High AR or SR may be useful in predicting the risk of rupture for intracranial aneuyrsms. But now, no definitive AR or SR level are accepted in discriminating between ruptured and unruptured aneurysms.Besides,multivariable binary logistic regression analysis show that AR/SR/Diameter of parent artery have independent relationships with aneurysms ruptured.
