Objective:In this study,the expression of lncRNA CoroMarker in peripheral blood mononuclear cells(PBMCs)and the expression level of monocyte chemoattractant protein-1(MCP-1)in plasma of patients with acute ST-segment elevation myocardial infarction(STEMI)were detected to analyze the value of CoroMarker in diagnosis and prognosis evaluation of STEMI patients and its possible mechanism in STEMI.Methods:1.A total of 30 patients with acute ST segment elevation myocardial infarction and percutaneous coronary intervention(PCI)were selected in the North Guangdong people’s Hospital affiliated to Shantou University from July 2020 to October 2020.15 ml venous blood was drawn immediately after admission,PBMCs and plasma were extracted and placed in the-80 ℃ cryogenic refrigerator for centralized detection.In addition,30 cases of non-coronary heart disease volunteers were collected as control group.15 ml of venous blood was collected,and the PBMCs and plasma were extracted and put in the-80 ℃ cryogenic refrigerator for centralized detection.The expression level of CoroMarker in PBMCs of the two groups was detected by fluorescence quantitative PCR,and the concentration of MCP-1 in plasma of the two groups was detected by ELISA.2.The difference of CoroMarker table level between the experimental group and the control group was compared,and the diagnostic value of CoroMarker in STEMI was analyzed.3.The correlation between CoroMarker and MCP-1 was analyzed,and the possible mechanism of Cormarker in STEMI was discussed.4.The patients were followed up for 6 months after discharge,and major adverse cardiovascular events(MACEs)were recorded to analyze the correlation between the expression of CoroMarker and the occurrence of MACEs,and to explore the value of coromarker in evaluating the prognosis of STEMI.Results:1.The expression of lncRNA CoroMarker in PBMCs of STEMI group was significantly lower than that of control group.The area under the ROC curve of STEMI diagnosed by CoroMarker was 0.781,and the Youden index was the highest when the CT value was 26.47,with a sensitivity of 66.7% and a specificity of 83.3%.2.The patients were regularly followed up for 6 months after discharge to analyze the correlation between the level of CoroMarker table and the occurrence of major adverse cardiovascular events(MACE).Logistic regression analysis showed that CoroMarker was one of the main factors for independent prediction of MACE within 6months.3.In STEMI group,The concentration of plasma MCP-1 was significantly higher than that in the control group.In STEMI group,the expression level of CoroMarker in PBMCs was negatively correlated with plasma MCP-1 concentration.Conclusion:1.The expression of lncRNA CoroMarker in peripheral blood monocytes of patients with STEMI decreased,while the expression of plasma MCP-1 increased in patients with STEMI.There was a significant negative correlation between the expression of CoroMarker and the concentration of plasma MCP-1.CoroMarker may be used as a biomarker of STEMI and may play a role in predicting MACEs with STEMI within 6 months.2.LncRNA CoroMarker may regulate the expression of MCP-1 through NF-κ B signal pathway,promote the adhesion of monocytes and vascular endothelial cells,and participate in the occurrence and development of myocardial infarction.
