| Objective: The purpose of this retrospective study was to evaluate the clinical effect of osteotome sinus floor elevation technique with simultaneous implant placement and concentrated growth factors alone by observing the changes of various reference indexes in different periods.Explore the osteogenic potential of CGF and provide theoretical basis for clinical operation.Methods:Clinical cases with osteotome sinus floor elevation(OSFE)implantation due to maxillary posterior tooth loss were collected from June2018 to June 2020 in the Stomatological Hospital of Hebei Medical University.Subjects were determined according to the inclusion criteria,and a total of 70implants(53 patients)were included in this study.The patients were divided into 4 groups according to different implantation materials of the sinus floor after maxillary sinus elevation.Group: only implanted with CGF,18 pieces;Bio-Oss group: implanted only Bio-Oss bone meal,16 pieces;Mixed group:implanted CGF+ Bio-Oss bone meal mixture,18 pieces;Blank group: pure OSFE without any graft material,18 pieces.The medical history of all patients was reviewed,and the general clinical data,such as gender,age,smoking,periodontal status and distribution of implant sites,were compared between the four groups.The imaging data of the four groups were measured immediately after operation,at 1 month,3 months and 6 months.The residual bone height,the implant protrusion length,survival rate,the endo-sinus bone gain,the peri-implant bone height,and the marginal bone loss were measured.Statistical analysis was conducted on the obtained data.Results: There was no statistical difference in general clinical data,the residual bone height and the implant protrusion length among the four groups,so that the four groups were comparable.1.There were no significant differences in the incidence of postoperative complications(all 0%),survival rate of implants(all 100%),and the marginal bone loss among the four groups;2.The endo-sinus bone gain and the peri-implant bone height: there were no statistical differences among the CGF group,the Bio-Oss group and the mixed group,which were better than the blank group(P < 0.05);3.The bone resorption rate: mixed group(17.2%)< CGF group(17.8%)< Bio-Oss group(21.9%)< blank group(28.3%).Conclusions:1.CGF may have good osteogenic potential,and CGF alone has a good bone augmentation effect on the OSFE.2.The low replacement rate bone graft material(Bio-Oss)can maintain the spatial configuration of the maxillary sinus,and the combined application with CGF can reduce the bone resorption in the sinus.3.OSFE without bone grafting has no effect on the survival rate of implants,but the amount of new bone is low. |
