Enteral Administration Of Glutamine To Prevent Necrotizing Enterocolitis In Preterm Infants:A Meta Analysis

ObjectiveTo systematically evaluate the effect of enteral administration of glutamine on the prevention of necrotizing enterocolitis in premature infants.MethodsWe search China Biology Medicine disc,Wanfang data,CNKI,VIP,PubMed,Cochrane Library,Web Of Science and EMBASE,to collect randomized controlled trials of glutamine in premature infants by computer system.The retrieval time was up to July 2020.After screening the literature according to the inclusion and exclusion criteria and extracting the data,the quality of the literature was objectively evaluated,and the Review Manager 5.3 software was used to conduct a Meta analysis of the included trial results.ResultsA total of 267 related literature were retrieved.On the basis of the inclusion criteria and exclusion criteria,11 randomized control trials were finally included,and a total of 1130 subjects were included.The results of Meta-analysis showed that there were statistically significant differences in the incidence of necrotizing enterocolitis(OR=0.29,95%CI: 0.17~0.49,P <0.00001)and necrotizing enterocolitis-related mortality(OR=0.12,95%CI: 0.02~0.68,P=0.02)between the glutamine group and the control group.Compared with the control group,the time to total enteral feeding(MD =-4.61,95% CI:-7.92~-1.30,P = 0.006)and hospital stay(MD =-4.80,95% CI:-8.66~-0.94,P = 0.01)in the glutamine group were significantly different.ConclusionThis study suggests that enteral administration of glutamine is effective in preventing the occurrence of necrotizing enterocolitis in premature infants and reducing its mortality,and it is also effective in shortening the time to total enteral feeding and hospital stay.Due to the limited literature of randomized controlled trials included in this study,factors such as the quality of the research literature,possible heterogeneity and publication bias were considered.In the future,more samples and higher quality of randomized controlled trials literature are needed to confirm the conclusion.The current research results are only for clinical reference.
