Clinical Application Of Perineal Ultrasound-guided Transperineal Prostate Biopsy

Abstract/Summary:
Objective:By comparing the diagnostic positive rate and complication rate of perineal ultrasound-guided transperineal and rectal ultrasound-guided transrectal prostate biopsy.The diagnostic value and safety were evaluated.To provide reference for clinicians to search a suitable biopsy route.Methods:The clinical data of 299 patients who came to the Department of Urology or Ultrasound Medicine for prostate biopsy from February 2019 to December 2020were retrospectively analyzed.All cases were confirmed pathologically.Among them,111 cases were diagnosed by transperineal prostate biopsy and 188 cases were diagnosed by transrectal prostate biopsy.The pathology results and postoperative complications of the two groups were collected.Then comparing the positive rate of biopsy and normal tissue complication probability among the two methods.Results:1.The positive rate of biopsy of transperineal and transrectal prostate biopsy:The overall positive rates of transperineal and transrectal prostate biopsy were 33.3%and 39.9%,respectively.There was no significant difference in the overall positive rates between the two groups(P>0.05).According to the PSA level,the PB positive rate of the two groups was compared,and the results showed that there was no significance difference in the positive rate under different PSA levels(P>0.05).The single-needle positive rate of transperineal and transrectal prostate biopsy was 21.7%and 24.4%,respectively.There was no significant difference in the single-needle positive rate between the two groups(P>0.05).The single-needle positive rate of PB in the two groups was compared after grouping according to puncture site,and the results showed that there was no significance difference in the single-needle positive rate of each puncture site(P>0.05).2.The complication rate of transperineal prostate biopsy of transperineal and transrectal prostate biopsy:The complication rate of transperineal prostate biopsy was significantly lower than that of transrectal prostate biopsy(49.5%vs 80.3%,χ~2=30.835,P<0.001).Among them,infection(37.8%vs 61.2%,χ~2=15.236,P<0.001),hematuria(14.4%vs 34.6%,χ~2=14.361,P<0.001),bloody stools(0%vs24.5%,χ~2=32.098,P<0.001)),and urinary retention(0.9%vs 8.0%,χ~2=6.903,P<0.05)of transperineal prostate biopsy was significantly lower than that of transrectal prostate biopsy,and the differences were statistically significant.Conclusions:1.Ultrasound-guided transperineal and transrectal prostate biopsy are both effective methods for the diagnosis of prostate disease.The two methods of biopsy are comparable in the diagnosis of prostate disease2.The perineal prostate biopsy performs better in terms of safety.3.Perineal ultrasound guided transperineal prostate biopsy is simple requirements for equipment,and it does not require intestinal preparation.Furthermore,there is no need for prophylactic antibiotics.So perineal ultrasound guided transperineal prostate biopsy is suitable for patients who cannot receive transrectal ultrasound.Doctors should consider the selection of puncture methods based on the hospital facilities,the proficiency of the operator,and the individual’s own conditions.
