spectrum curve in the evaluation of the differentiation degree of gastric cancer. Methods: 98 cases of gastric cancer diagnosed as non mucinous cancer by gastroscopy were collected in our hospital. The CT value, iodine base value, effective atomic number, water base value and slope of energy spectrum curve were analyzed. Results: There was no significant difference in CT values of poorly differentiated and moderately differentiated gastric cancer with different energy levels in the arterial phase(P>0.05);the CT values of poorly differentiated gastric cancer at 40,50,60,70,80,90 ke V in the venous phase were respectively Are(301.3±3.4),(238.5±3.4),(189.6±3.2),(150.6±2.4),(120.6±2.6),(98.6±3.4)Hu,which are higher than those of moderately and well differentiated(195.3±23.4)),(141.6±2.4),(101.6±3.3),(81.6±2.4),(70.2±2.6),(60.3±1.8)Hu,the difference was statistically significant(P<0.05);parenchymal stage 40,50 The CT values of poorly differentiated gastric cancer at,60,70,80,and 90 ke V were(289.3±3.4),(218.5±5.6),(193.6±2.8),(143.6±2.4),116.6±2.6),(75.6±3.4),respectively Hu,which is higher than that of moderately and well differentiated(172.3±13.4),(132.6±2.4),(98.6±3.9),(78.6±2.4),(67.2±2.6),(56.3±1.8)Hu,the differences are all It was statistically significant(P<0.05);there was no statistically significant difference in CT values of poorly differentiated gastric cancer with venous and parenchymal stages of 100-140 ke V compared with moderately and poorly differentiated(P>0.05).The iodine base value, effective atomic number, water base value and energy spectrum slope of patients with poorly differentiated arterial stage gastric cancer were not statistically different from those of moderately and well-differentiated patients(P>0.05);Iodine in patients with poorly differentiated gastric cancer at venous stage and parenchymal stage The base value, the slope of the energy spectrum curve and the effective atomic number are higher than those of moderately and well-differentiated,and the difference is statistically significant(P<0.05);there is no difference between the poorly differentiated water base value of venous and parenchymal gastric cancer and moderately and well-differentiated patients Statistically significant(P>0.05).Conclusion: There is a significant correlation between the energy spectrum curve of gastric cancer lesions and the degree of differentiation of patients’ tissues.Gem CT energy spectrum curve can reflect the degree of differentiation of patients with gastric cancer, and the degree of differentiation of patients can be evaluated by analyzing the iodine base.Part Ⅱ Value of energy spectrum CT volumetric iodine content on the effect of chemotherapy in advanced gastric cancerObjective: To study the energy spectrum CT volumetric iodine content examination in patients with advanced gastric cancer chemotherapy and evaluate the effect of chemotherapy. Methods: 98 patients with advanced gastric cancer treated in our hospital were selected as the time range of this study. All patients were treated with conventional chemotherapy.51 patients were effective and 47 patients were ineffective. All patients underwent spectral CT volumetric iodine content examination. The relevant parameters and iodine concentration of patients before and after treatment were compared. Results: There was no statistically significant difference in related parameters of patients with advanced gastric cancer chemotherapy before treatment(>0.05).The maximum tumor diameter, maximum tumor thickness, maximum tumor crosssectional area, and TV value of the effective group after treatment were lower than those of the reference group, and the difference was statistically significant Academic significance(<0.05).There was no statistically significant difference in iodine concentration in patients with advanced gastric cancer chemotherapy before treatment(>0.05).After treatment, the effective group’s iodine value of arterial and venous tumors/aortic iodine value of the same layer and the average iodine concentration of lesions were lower than those of the ineffective group. There is statistical significance(<0.05).Conclusion: The effect of chemotherapy in patients with advanced gastric cancer has high accuracy through energy spectrum CT volumetric iodine content examination, which can be evaluated by its tumor condition and iodine concentration level.Part Ⅲ The clinical value of CT spectrum imaging in preoperative assessment of pathological types of patients with gastric cancerObjective : To explore the clinical value of CT energy spectrum imaging in preoperative assessment of the pathological types of patients with gastric cancer. Methods: 121 cases of gastric cancer patients in our hospital were selected for CT energy spectrum imaging. The water and iodine concentrations of lesions were measured, and the standardized iodine concentration ratio was calculated. The iodine concentration, iodine concentration ratio and water concentration levels of different pathological types were analyzed and compared. Results: The iodine concentration and the ratio of iodine concentration in the venous and parenchymal stages of gastric mucinous carcinoma were lower than those of gastric non-mucinous carcinoma patients, and the differences were statistically significant(P<0.05);the arterial, venous, and parenchymal gastric non-mucinous carcinoma patients There was no significant difference in the ratio of water concentration to arterial iodine concentration and iodine concentration in patients with gastric mucinous carcinoma(P>0.05).The iodine concentration and the ratio of iodine concentration in the venous stage and parenchymal stage of mucinous adenocarcinoma patients were lower than those of patients with papillocarcinoma, and the differences were statistically significant(P<0.05);patients with mucinous adenocarcinoma had levels of arterial, venous, and parenchymal stages. There were no statistically significant differences in the ratio of iodine concentration to arterial iodine concentration, and iodine concentration compared with patients with print cell carcinoma(P>0.05).The iodine concentration and the ratio of iodine concentration in the venous stage and the parenchymal stage of patients with moderately and well differentiated adenocarcinoma were lower than those of patients with poorly differentiated adenocarcinoma, and the differences were statistically significant(P<0.05);patients with moderately well differentiated adenocarcinoma in arterial, venous, and parenchymal stages There was no statistically significant difference in the ratio of water concentration, arterial iodine concentration, iodine concentration and poorly differentiated adenocarcinoma patients(P>0.05).Conclusion: CT spectroscopic imaging plays an important role in the preoperative evaluation of patients with gastric cancer. The pathological types of gastric cancer are different, and the iodine concentration will change accordingly.CT spectroscopic imaging can effectively evaluate the pathological types of gastric cancer, which is beneficial to the clinical treatment guidance of patients, and has a very high performance. The value of clinical application. Therefore,in the assessment of pathological types of gastric cancer patients,CT energy spectrum imaging should be further promoted and applied. |
