The Effect Of Postoperative Effect Of Chronic Sinusitis Patients With Abnormal Middle Turbinate Who Received Different Kinds Of Trentment

Objective:Investigate the effect of efficacy of CRSsNP patients who accepted different therapies of abnormal middle turbinate when they got nasal endoscopic surgery treatment.Methods:1.Case collection: I have selected with a diagnosis of CRSsNP with abnormal middle turbinate structures who were treated with nasal endoscopic sinus surgery,in our hospital between December 2018 and December 2019.The patients included in this study with continuous or discontinuous clinical symptoms more than 3 months who accorded with the diagnosis and therapy standard published in 2018.They had been diagnosed with CRSsNP by anterior rhinoscope,nasal endoscopic and CT examination lens,age range:18-65 years old.Got no less than 12 weeks of standard drug treatment,they still had the symptoms such as rhinobyon,festering stuff and hyposmia.These patients were bilateral lesions.All included patients agreed to the study and signed informed consent.They were randomly invided into a observation group of 52 patients and a control group of 51 patients according to the hospital sequence by random number table.The patients of observationg group received middle turbinate angioplasty cures and the other group’s patients received partial resection of middle turbinatecures.2.Treatment and Methods: The patients of observation group were treated with middle turbinate angioplasty.(1)The vesicular middle turbinate needs to cut from the middle with turbinate scissors,separate bilateral mucosa and bone with attractor stripper,cut the bone and be against the inboard bone and outboard mucosa.(2)The hypertrophy middle turbinate needs to cut open the mucous membrance from the leading edge with sickle knife,in order to form a normal middle turbinate,cut hypertrophy part of bone,then turned up the medial mucosa longer than bone and connected with the lateral wound.(3)The middle turbinate of reverse deviation needs to fracture local deviation of middle turbinate to restore normal anatomical position.The patients of control group were treated with partial resection of middle turbinate.It needs to cut the perpendicularis part of middle turbinate with punch forceps.The lower boundary of turbinate flush with the upper limb of maxillary ostium.In order to guarantee the feasibility of the research,the operation was completed by the same doctor.3.Evaluation of therapeutic effect: Through comparation of VAS,SNOT-22 and Lund-kennedy scores and the condition of complication before operation cures and after operation cures 1 year,the effect of effect after operation cures of two kinds of treatment were compared.4.Data processing: SPSS 24.0 statistical software was used for all data analysis,in which χ2 test was used for counting data.The measurement data was tested by t test.P<0.05 was considered statistically significant.Results:1.Comparison of age,sex,course of disease and types of medial turbinate structural abnormalities of two groups’ patients was no significant difference,P>0.05.2.Comparison of VAS,Lund-Kenney,Lund-Mackay and SNOT-22 of two groups before operation was no significant difference,P>0.05.3.The scores of pus stuff,nasal congestion,general malaise,headache and SNOT-22 of the observation group’s patients after treatment 1 year were obviously lower than preoperative scores,P<0.05.The improvement of olfaction was not obvious,P=0.35.4.The scores of pus stuff,nasal congestion,general malaise,headache and SNOT-22 of the control group’s patients after treatment 1 year were obviously lower than preoperative scores,P<0.05.The improvement of olfaction was not obvious,P=0.44.5.The scores of pus stuff,nasal congestion and headache of the observation group’s patients after treatment 1 year were obviously lower than that of control group,P<0.05.The scores of olfaction and general malaise were no significant difference,P>0.05.6.The scores of Lund-Kenney、SNOT-22 of the observation group’s patients observation group’s patients were obviously lower than that of control group,P<0.05.7.The number of observation group’s complication was were obviously lower than that of control group,P<0.05.There was no patients recur after treatment 1year.Conclusion:For CRSsNP patients with abnormal middle turbinate structures line nasal endoscopic sinus surgery and middle turbinate in operation,can effectively reduce the occurrence of complications,improve patients postoperative efficacy and satisfaction.
