| Objective: Analysis of additional radiation therapy and the chemo-radiation,pure to cervical cancer radical surgery in only merge moderate dangerous factors in the curative effect of midlife and senile pa-tients,and the prognostic factors of midlife and senile patients with cervical cancer were come into question.Methods: A total of 132 middle-aged and elderly patients with c ervical cancer who were admitted to the gynecological ward of our h ospital for the first time from January 2010 to June 2015 were colle cted.All of them underwent radical laparotomy for cervical cancer,a nd only moderate risk factors were found in postoperative pathologica l results.Among the 132 patients,60 received postoperative radiothera py and 72 received concurrent chemoradiotherapy.The 5-year survival of the two groups of patients was compared,and the effects of differ ent adjuvant treatment regimens and different clinicopathological facto rs on the prognosis were analyzed.Results: The 5-year overall survival rate was 65.00% in the grou-p of radiotherapy alone and 81.94% in the group of concurrent chem-oradiotherapy,respectively.The 5-year progression-free survival rate was 63.33% in the group of radiotherapy alone and 79.17% in the g-roup of concurrent chemoradiotherapy,respectively.The difference wa-s statistically significant(P=0.027,0.004).The 5-year progressionfree survival was 47.06% in the group with ≥2 intermediate risk factors after radiotherapy alone,and 70% was the 5-year overall survival rate,all of them were lower than those of the postoperative concurrent chemoradiotherapy group(52.94% and 80%).The difference was statistically significant(P=0.039,0.013).In the process of the follow-up,the proportion of distant metastasis in the postoperative radioth-erapy group was 25.00%,which was higher than that in the postope-rative concurrent chemoradiotherapy accounted for 11.11%,the differe-nce was statistically significant(P=0.031).The course of treatment,simple radiotherapy group occurred myelosuppression ratio of 43.33%,less than 63.77% of the postoperative concurrent chemoradiation grou-p.the difference was statistically significant(P=0.008).Through mu-ltivariate analysis,lymphatic vascular invasion,cervical interstitial inf-iltration ≥1/2,the number of middle-risk element ≥2,are the indep-endent element influencing the prognosis of midlife and senile cervi-cal cancer.Conclusion:1.Pure has the middle-risk element in the midlife and senile sic-k persons with cervical cancer radical surgery,adding chemoradiation compared with radiotherapy alone for application,can obtain higher survival rate.Especially in patients with intermediate risk factors ≥2,the effect is significant,and concurrent chemoradiotherapy can be gi-ven priority in treatment.2.The presence of lymphatic vascular invasion,interstitial invasi-on ≥1/2,and moderate risk factors ≥2 are independent element in fluencing the prognosis of patients with cervical cancer with moderate risk factors alone.3.Patients receiving concurrent chemoradiotherapy had a higher i-ncidence of myelosuppression during treatment than patients receiving radiotherapy alone,but the side effects were within the patient’s tol-erance range. |
