Objective:To observe the clinical effect of the prophylaxis treatment of migraine with Dispersing Stagnant Liver-Qi and Calming spirit,and to provide an effective method for clinical treatment.Methods:Seventy patients who met the inclusion criteria and participated in the trial were collected from the 5th Department of Acupuncture and Moxibustion in the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from December 2019 to January 2021,and were randomly divided into treatment group and control group with 35 patients in each group according to a random number table.The treatment group was treated with Dispersing Stagnant Liver-Qi and Calming spirit acupuncture,which was combined with penetration acupuncture,scalp acupuncture and conventional acupuncture.Acupuncture was performed once every other day,and the needles were kept for 30 min.2 weeks was a course of acupuncture,and there were 2 courses of acupuncture in total.The control group was initially given 5mg flunarizine hydrochloride,once a day before sleeping orally.If there was no obvious adverse reaction within 1 week,the dosage was added to 10 mg,orally taken once a day before sleeping.The treatment lasted for 4 weeks.The results included headache severity--migraine symptom score,total response rate,visual analogue scale of pain intensity(VAS),migraine attack frequency,migraine specific quality of life questionnaire(MSQ),14 versions of Hamilton Anxiety Scale(HAMA)and 24 versions of Hamilton Depression Scale(HAMD).SPSS21.0 statistical software was used to analyze the data and compare the results and observe the difference of curative effect between the two groups.Results:1.Of the 6 patients who fell off during the experiment,64 cases actually completed the experiment,including 31 cases in the treatment group and 33 cases in the control group.Before treatment,there were no significant differences in general condition(age,gender,course of disease,diagnosis),score of migraine symptom,VAS,frequency of migraine attack,MSQ,HAMA and HAMD between the two groups(P > 0.05),indicating comparability.2.Clinical efficacy: The total effective rate of the treatment group was96.8%,which was better than that of the control group(87.9%),with statistically significant difference(P < 0.05).3.Migraine symptom score: After treatment,the score of migraine symptom in both groups was significantly decreased(P < 0.01),and the score of migraine symptom in the treatment group was lower than that in the control group,with statistically significant difference(P < 0.01).4.VAS: After treatment,VAS scores in both groups were significantly decreased(P < 0.01),and the VAS scores in the treatment group were significantly lower than those in the control group,with statistically significant differences(P <0.01).VAS scores in experimental groups were lower than those in control group during the follow-up period(P < 0.01).5.Frequency of migraine attack: After treatment,the frequency of migraine attack in the two groups was significantly decreased(P < 0.01),and the frequency in the treatment group was lower than that in the control group,with statistical difference(P < 0.05);During the follow-up period,the frequency of migraine attack in experimental groups was lower than that in control group(P< 0.05).6.MSQ score: After treatment,the MSQ score of the two groups decreased significantly(P < 0.01).The score of functional limitation and dysfunction in the treatment group was significantly better than that in the control group(P <0.01),and the score of emotional influence in the treatment group was significantly better than that in the control group(P < 0.05).7.HAMA score: After treatment,the HAMA score of both groups was significantly decreased(P < 0.01),and the score of the treatment group was significantly lower than that of the control group,with statistically significant difference(P < 0.01).8.HAMD score: After treatment,the HAMD score of both groups was significantly decreased(P < 0.01),and the score of the treatment group was significantly lower than that of the control group,with statistically significant difference(P < 0.01).9.No adverse reactions occurred in the Dispersing Stagnant Liver-Qi and Calming spirit acupuncture group during the treatment process,while 1 case of drowsiness was reported in the flunarizine hydrochloride group during the treatment process,with no serious adverse reactions.Conclusion:1.Dispersing Stagnant Liver-Qi and Calming spirit acupuncture and flunarizine hydrochloride prophylactic treatment of migraine has a clear effect.2.Dispersing Stagnant Liver-Qi and Calming spirit acupuncture method is better than flunarizine hydrochloride group in alleviating headache intensity,and also has better efficacy in the follow-up period;At the follow-up of 4 weeks,the efficacy of the Dispersing Stagnant Liver-Qi and Calming spirit acupuncture group continued to improve,the efficacy of the flunarizine hydrochloride group was stable,and the efficacy of both groups began to decline at the follow-up of 8 weeks.3.Dispersing Stagnant Liver-Qi and Calming spirit acupuncture is better than flunarizine hydrochloride group in reducing the frequency of migraine attacks,and has better efficacy in the follow-up period;The efficacy of the Dispersing Stagnant Liver-Qi and Calming spirit acupuncture group was stable at 4 and 8 weeks of follow-up,the efficacy of the flunarizine hydrochloride group was stable at 4 weeks of follow-up,but decreased at 8 weeks of follow-up.4.The efficacy of Dispersing Stagnant Liver-Qi and Calming spirit acupuncture in improving patients’ quality of life was significantly better than that of the flunarizine hydrochloride group in terms of functional limitation and dysfunction,and was better than that of the flunarizine hydrochloride group in terms of emotional influence.5.The therapeutic effect of Dispersing Stagnant Liver-Qi and Calming spirit acupuncture in improving patients’ mood disorders was significantly better than that of flunarizine hydrochloride group.6.Dispersing Stagnant Liver-Qi and Calming spirit acupuncture is an effective method for prophylactic treatment of migraine,and the clinical effect is better than flunarizine hydrochloride.
