Retrospective Study On Sequential And Consolidation Therapy Of Traditional Chinese Medicine In The Treatment Of Advanced Primary Liver Cancer

Objective:In this study,retrospective analysis was used to analyze the effects of sequential treatment of traditional Chinese medicine and consolidation treatment of traditional Chinese medicine(TCM)on the survival of patients with primary liver cancer,related TCM syndromes and laboratory indicators.Methods:According to the inclusion and exclusion criteria,patients with primary liver cancer(PLC)were collected from the outpatient department of oncology,the First Affiliated Hospital of Heilongjiang University of Chinese Medicine from January 1 st,2013 to January 1st,2018.The record includes the general condition of the patient(age of onset,gender,drinking status,past medical history,family history),Chinese medicine situation(first-diagnosed symptoms,first-diagnosed TCM syndrome type),Western medicine situation(tumor pathological type,TNM staging,metastasis site),Treatment status(systemic chemotherapy,radiotherapy,targeted therapy,interventional therapy and other Western medical treatments),treatment modes(sequential treatment with TCM and consolidation treatment with TCM),Use of traditional Chinese medicine,overall survival time,etc.Use SPSS to conduct retrospective analysis,compare the differences in the survival time,TCM syndromes and laboratory indicators of patients with primary liver cancer under the two treatment modes.Result:1.A total of 159 patients with PLC were treated in the outpatient department of the Oncology Department of the First Affiliated Hospital of Heilongjiang University of Chinese Medicine.Among them,80 were in the sequential treatment with TCM group and 79 were in the consolidation treatment with TCM group.The baseline level of the two groups were balanced,and the general situation of the patients was not statistically significant(P<0.05).2.In the sequential treatment with TCM group,the mean survival time was 11.98±6.81 months,the median survival time(MST)was 11 months,and the 6,12,18,24months survival rates was 68.75%,43.75%,13.75%,6.25%respectively.While in the consolidation treatment with TCM group,the mean survival time was 9.71±4.71 months,the MST was 10 months,and the 6,12,18,24months survival rates was 67.09%,31.65%,2.53%,1.27%respectively.The mean survival time of patients in sequential treatment with TCM group was longer than that in the consolidation treatment with TCM group,with significance statistic differences(P<0.05),but there was no differences between the two groups in survival rate(P>0.05).3.The symptoms of fatigue,lack of appetite,abdominal distension and insomnia in the sequential treatment with TCM group were significantly improved after treatment(P<0.01),and the hypochondriac pain was improved after treatment(P<0.05).After treatment,the symptoms of fatigue and lack of appetite in the consolidation treatment with TCM group were significantly improved(P<0.0 1),and Insomnia was relieved after treatment,with statistical significance(P<0.05).There was no significant relief in hypochondriac pain and abdominal distension after treatment(P>0.05).Compared with the two groups,the symptoms of fatigue,lack of appetite,abdominal distension and insomnia were statistically significant after treatment(P<0.05),but there was no significant relief in the symptoms of hypochondriac pain in both two group(P>0.05).4.After treatment,the number of effective,stable and ineffective cases in the sequential treatment with TCM group were 29 cases(36%),39 cases(49%)and 12 cases(1 5%)respectively,and the number of effective,stable and ineffective cases in the consolidation treatment with TCM group were 23 cases(29%),40 cases(5 1%)and 16 cases(20%)respectively.For the effective rate and stability rate,there has little differences between the two groups(P>0.05).5.The sequential group with TCM showed a significant decrease in ALT and TBIL,which was statistically significant compared with the previous(P<0.01),AST and ALB were not significantly improved(P>0.05).In the consolidation group with TCM,TBIL decreased significantly,which was statistically significant compared with the previous(P<0.01),AST,ALT and ALB were not significantly improved than before treatment(P>0.05).There was no significant differences in AST,ALT,TBIL and ALB between the two groups after treatment(P>0.05).Conclusions:1.Sequential treatment with TCM has advantages in prolonging the average survival and median survival of patients with advanced primary liver cancer compared with consolidation treatment with TCM;However there was no significant difference in the survival rate of 6,12,18,24 months between the two groups.2.The two treatments have a certain effect on the improvement of TCM syndromes,especially in the aspects of fatigue and lack of appetite.Compared with consolidation treatment with TCM,sequential treatment with TCM has more advantages in improving TCM syndrome;But there was no significant difference in the improvement of hypochondriac pain between the two groups.3.After treatment,the level of AFP decreased in both two groups,but there was no significant differences in the effective rate and stability rate.4.The liver function of the patients in the two groups improved to varying degrees after treatment,especially the improvement in TBIL;But,there is no significant difference between the sequential treatment with TCM and the consolidation treatment with TCM in improving liver function.
