Objectives: The lobectomy and systematic nodal dissection is the standard treatment for non-small cell lung cancer(NSCLC)and routine dissection of the superior interlobular lymph nodes(11s nodes)is not required for tumors in the middle or lower lobes of the right lung.The goal of this study was to examine the metastasis and high-risk factors of #11s lymph nodes.Methods: 157 patients with NSCLC in the middle or lower lobe of the right lung underwent lobectomy and systemic lymph node dissection along with dissection of 11 s node.64 patients showed 11 s lymph node invasion,and were divided into 11s(+)and 11s(-).Results: There were 31 cases of lymph node invasion in the 11 s nodes,and the overall incidence of metastasis was 19.75%,including 13.64% for middle lobe tumors and 20.74% for lower lobe lung cancers.the factor associated with 11 s node metastasis was 2r+4r nodes involvement(p<0.05).The #7 nodes(p<0.05)and the inferior mediastinal lymph nodes involvement(p<0.05)are high-risk factors affecting the prognosis of patients.The 11 s nodes metastasis has nothing to do with the location of the tumor,and it is not an independent factor affecting disease-free survival.Conclusion: The 11 s nodes may be a transit station for 2r+4r lymph node metastasis in right middle or lower lobe lung cancer,and the 11s#group should be cleared in the surgical treatment of NSCLC in either the middle or lower lobe of the right lung,and the clinical significance regarding the clearance of lymph nodes in this group deserves further study.The influencing factors of disease-free survival after surgery for lung cancer in the right middle or lower lobe are the metastasis of the subcarinal lymph nodes and the inferior mediastinal lymph nodes.
