Objective:To observe the clinical efficacy of myofascial trigger point drying needling combined with PNF stretching technique on patients with chronic nonspecific low back pain,and provide a new method for the treatment of chronic nonspecific low back pain.Methods:50 patients with chronic nonspecific low back pain in Rehabilitation center of Guangzhou Sport University from June 2019 to December 2020 were randomly divided into experimental group and control group,25 cases in each group.The experimental group was treated with myofascial trigger point dry needling therapy combined with PNF stretching,once a week,for 4 weeks.Myofascial trigger point were marked at the gluteus medius,quadratus psoas,gluteus maximus and erector spinae muscle successively.The silver needle 0.30 mm × 75 mm was used to quickly enter the bone surface,and the needle was slightly withdrawn to the muscle layer for up and down lifting and insertion.The patients in the control group were treated with electroacupuncture combined with PNF stretching,3 times a week,for 4weeks.According to the "low back pain: evidence based acupuncture clinical practice guide",the acupoints were marked,and the acupuncture was carried out with the method of flat reinforcing and flat reducing.After the feeling of De Qi appeared,the wire of the electroacupuncture therapeutic apparatus was connected with the tail end of the filiform needle,adjusted to the density wave mode,and the power was set to 2Hz for 20 minutes.The visual analog scale(VAS),lumbar disability index(ODI),pain pressure threshold(PPT)at the trigger point and the overall effective rate of the two groups were observed before treatment,after treatment and 1 month after treatment.Spss24.0 software was used for statistical data analysis,,the difference was statistically significant(P<0.05).Results:1.VAS score:The VAS score of the experimental group in the treatment of 4weeks,4 weeks follow-up compared with before treatment,the difference was statistically significant(P < 0.05).The VAS score of the control group in the treatment of 4 weeks,4 weeks follow-up compared with before treatment,the difference was statistically significant(P < 0.05).After 4 weeks of treatment and 4 weeks of follow-up,the difference between the two groups was statistically significant(P <0.05)。2.ODI index:The ODI index of the experimental group in the treatment of 4weeks,4 weeks of follow-up compared with before treatment,the difference was statistically significant(P < 0.05).The ODI index of the control group in the treatment of 4 weeks,4 weeks follow-up compared with before treatment,the difference was statistically significant(P < 0.05).After 4 weeks of treatment,the difference between the two groups was statistically significant(P < 0.05)。3.Pressure pain threshold : PPT in the experimental group after 4 weeks of treatment,4 weeks of follow-up compared with before treatment,the difference was statistically significant(P < 0.05).PPT in the control group after 4 weeks of treatment,4 weeks of follow-up compared with before treatment,the difference was statistically significant(P < 0.05).After 4 weeks of treatment and 4 weeks of follow-up,the difference between the two groups was statistically significant(P < 0.05).4.Clinical efficacy comparison: The experimental group was followed up for 4weeks,the effective rate was 92%,the control group was followed up for 4 weeks,the effective rate was 64%,the experimental group was higher than the control group.5.Comparison of adverse reactions: There were 4 cases of grade II adverse reactions in the experimental group and 2 cases of grade II adverse reactions in the control group.the difference was not statistically significant(P > 0.05).Conclusion:1.Both dry needling at the trigger point of the gluteus muscle combined with PNF stretching,and the traditional electroacupuncture group combined with PNF stretching can alleviate the symptoms of chronic low back pain in the short term.2.Myofascial trigger point dry needling combined with PNF stretching group can significantly relieve the symptoms of chronic nonspecific low back pain in a short time,and the effect is better than that of traditional electroacupuncture combined with PNF stretching group.3.The clinical safety of dry needling at the trigger point of gluteus muscle and traditional electroacupuncture in the treatment of chronic low back pain is good.
