Research Purpose:1.Fundamental Purpose:Through a randomized controlled study,we attempted to observe the effects of acupuncture combined with traditional Chinese medicine on the endometrial receptivity,pregnancy rate and quality of life of patients with thin endometrial infertility,so as to explore an effective treatment scheme of acupuncture combined with traditional Chinese medicine for this disease.2.Sencordary Objecyive:To compare the therapeutic effect difference between acupuncture combined with traditional Chinese medicine and sequential therapy of estrogen progesterone.The research content:The first part:literature research Review A:Modern medical overview of thin endometrial infertility:Through information retrieval on CNKI,Wanfang,Weipu and other Chinese academic journal databases,as well as Sci-Hub,Pub-Med,Springer Link and other English academic resource platforms,the research progress on the pathogenesis and treatment of thin endometrium and infertility was summarized.Review B:Summary of TCM research on thin endometrial infertility:Through the literature review provided by the Chinese academic journal database above,the cognition,treatment and research status of TCM on thin endometrium and infertility were summarized.The second part:clinical research The research methods:A total of 85 infertility patients who were in line with the diagnosis of thin endometrium were collected from the outpatients and inpatients of the Second Affiliated Hospital of Anhui University of Chinese Medicine.These patients were divided into the experimental group(acupuncture and drug combination group,42 cases)and the control group(estrogen progesterone sequential group,43 cases)by a randomized controlled trial method.Endometrial receptivity indexes were checked regularly during the enrollment period,and couples were instructed to have sex according to the actual situation.After treatment,endometrial thickness,estrogen change level,menstrual volume increase and decrease,TCM syndrome score were observed,and follow up statistics of pregnancy rate and comprehensive efficacy were conducted for comparison in many aspects.Results:Endometrial thickness:In the experimental group,endometrial thickness increased after treatment,which was statistically significant(P<0.05).In the control group,endometrial thickness increased after treatment,which was statistically significant(P<0.05).After treatment,endometrial thickness in the control group was greater than that in the treatment group,with statistical difference(P<0.05).The increase of endometrial thickness in the experimental group and the control group was1.51±0.71 mm and 2.73±0.90 mm,respectively,which showed statistically significant difference(P<0.05).Endometrial morphology:In the experimental group,the proportion of type A endometrium increased significantly,while the proportion of type B and C endometrium decreased.In the control group,the proportion of endometrium of type A and type B increased before treatment,while that of type C decreased before treatment.After treatment,the group A type intima was higher than the control group.In terms of endometrium type B and type C,the experimental group was less than the control group.Uterine artery blood flow parameters:The uterine arterial pulse index in the experimental group decreased,which was statistically significant compared with that before treatment(P<0.05).The uterine arterial pulse index in the control group decreased slightly,but this was not statistically significant compared with that before treatment(P>0.05).After treatment,the value of uterine arterial pulse index in the experimental group was significantly lower than that in the control group,with a statistically significant difference(P<0.05).The uterine artery resistance index in the experimental group decreased after treatment,which was statistically significant compared with before treatment(P<0.05).The resistance index of uterine artery was slightly decreased in the control group,which was not statistically significant compared with before treatment(P> 0.05).After treatment,the value of uterine artery resistance index in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).Estrogen level:The peak day of estradiol measured before ovulation showed that E2 increased in both the experimental group and the control group,and the difference was statistically significant compared with before treatment(P < 0.05).The control group was higher than the experimental group,and there was statistical significance between the two groups(P < 0.05).Changes in menstrual volume:By comparing before and after treatment in each group,we found that the score of menstrual blood loss was significantly increased between the experimental group and the control group,which had a statistically significant difference(P<0.05).Compared with the control group,the experimental group showed a greater increase(P<0.05).TCM syndrome scores:The scores of TCM syndrome in the experimental group decreased after treatment,which was significant(P<0.05),while there was no significant statistical difference in the control group before and after treatment(P>0.05).After treatment,scores in the experimental group were lower than those in the control group,and the difference was statistically significant(P<0.05).The pregnancy rate:The pregnancy rates of the experimental group and the control group were 37.5% and 27.5%,respectively,without statistical significance(P>0.05).Comparison of clinical efficacy:The total effective rate of the experimental group was 87.5%,and the total effective rate of the control group was 72.5%,there was no statistical difference between the two.Conclusion:1.Acupuncture combined with traditional Chinese medicine can increase intimal thickness,improve intimal morphology,increase intimal blood perfusion,increase the level of estrogen in the body before ovulation,improve TCM syndromes,and improve intimal receptivity.2.Acupuncture combined with traditional Chinese medicine can promote pregnancy by stimulating potential activity of thin endometrium.
