| Background Breast cancer is a common malignancy in women and one of the main causes of cancer death.The incidence and mortality of breast cancer in China are expected to continue to rise for a long time in the future.Studies have shown that breast cancer can be detected,diagnosed and treated early through population screening,and early diagnosis can significantly improve the survival of patients.Objective To explore the value of S-Detect combined with elastography in differential diagnosis of benign and malignant breast nodules.Methods A total of 112 patients with breast nodules who were admitted to the Department of Ultrasound or the Department of Breast and Thyroid Surgery of the First People’s Hospital of Yichang from October 2019 to February 2021 were selected as the subjects.All patients were examined by two-dimensional ultrasound,Ultrasonic elastography(UE)and S-Detect technology,The ultrasound elasticity score was performed according to the modified 5-point method,and the BI-RADS classification given by two-dimensional ultrasound was classified and adjusted,by surgery or biopsy results as the gold standard,evaluation of BI-RADS classification,Elasticity score,S-Detect technology and jointly set the sensitivity(SE)of the diagnosis of benign and malignant breast nodules,specific(SP)and accuracy(ACC),positive predictive value(PPV),negative predictive value(NPV).ROC curves were used to evaluate the value of elastic strain rate ratio in different directions in diagnosing benign and malignant nodules of the breast and to obtain the optimal diagnostic threshold.According to the pathological results,the patients were divided into benign group and malignant group.The differences in ultrasonic characteristics(morphology,orientation,margin,internal echo,posterior echo feature,microcalcification),elasticity score and BI-RADS classification between the two groups were compared,and P < 0.05 was statistically significant.Results(1)In this study,surgical or puncture pathological results were the gold standard.Among 112 breast nodules,48 were malignant nodules and 64 were benign nodules.Among them,47 were malignant nodules and 65 were benign nodules diagnosed by BI-RADS.Elastic score diagnoses 49 malignant nodules,63 benign nodules,S-Detect technology diagnoses 50 malignant nodules,62 benign nodules,combined group diagnoses49 malignant nodules and 63 benign nodules.(2)The SE,SP,ACC,PPV and NPV of BI-RADS classification were 70.83%,79.69%,75.89%,72.34% and 78.46% for benign and malignant breast nodules.Elastography score for benign and malignant breast nodules was81.25% for SE,84.38% for SP,83.04% for ACC,79.59% for PPV and 85.71% for NPV.The SE,SP,ACC,PPV and NPV of benign and malignant breast nodules were 89.58%,89.06%,89.29%,86.00%,and 91.94%,respectively,with S-detect technology.The SE,SP,ACC,PPV and NPV of the combined group were 93.75%,93.75%,91.84% and 95.24% in benign and malignant breast nodules.(3)ROC curve results showed that when the elastic strain rate ratio(long axis)was set as the truncation value of 2.29,the diagnostic sensitivity was 81.3%,the specificity was 71.9%,and the AUC was 0.786(95%CI:0.701-0.872,P < 0.0001).The sensitivity and specificity of the diagnosis were 81.3% and 65.6% when the elastic strain rate ratio(short axis)was set as the cut-off value of 2.11.AUC was 0.783(95%CI: 0.6999-0.868,P < 0.0001).(4)The comparison of ultrasound imaging features of benign and malignant breast nodules showed that the comparison of benign and malignant breast nodules in morphology,orientation,edge,internal echo,posterior echo features,microcalcification in nodules,elasticity score,BI-RADS classification and other aspects had statistical significance(P < 0.05).Conclusion(1)The ratio of elastic strain rate has a certain value in distinguishing benign and malignant breast nodules.When the ratio of elastic strain rate is 2.29 on the long axis and 2.11 on the short axis,the diagnostic efficiency is the highest.(2)Conventional ultrasound BI-RADS classification,elastography,and S-Detect technology are all effective methods to identify benign and malignant breast nodules.(3)Compared with conventional ultrasound BI-RADS,S-Detect technology combined with elastography technology has a higher diagnostic efficiency for breast nodules and a higher detection rate for breast cancer,which can provide clinically valuable diagnostic evidence and reduce Unnecessary biopsy or surgery for breast nodules has good promotion and use value. |
