ObjectiveThe Breast Imaging Reporting and Data System(BI-RADS)classification of ultrasound proposed by the American College of Radiology(ACR)has standardized the classification of breast ultrasonic diagnosis and has been widely used.Ultrasound BI-RADS different classification levels suggest that there is a difference in the degree of malignancy of the lesions,and there is a relatively uniform evaluation standard for qualitative,so that a corresponding clinical management model is adopted for each classified lesion.If the ultrasound BI-RADS classification can better evaluate breast lesions,it can provide effective support for the accuracy of clinical diagnosis.In the ultrasound diagnostic examination,there are lesions in which the ultrasound BI-RADS classification is inconsistent with the pathological diagnosis.Often,such lesions may be over-treated or missed the best treatment period because they have not been diagnosed early or their corresponding clinical management.By comparing the ultrasound BI-RADS classification with the histopathological diagnosis of breast lesions,this study summarized the sonographic findings and histopathological types of the qualitatively inconsistent cases between the two,and explored the reasons for the inconsistency between the two,so as to more standardized the ultrasound BI-RADS classification and improve the coincidence rate of ultrasound diagnosis.MethodsFrom January 2017 to March 2019,all patients underwent breast ultrasound examination at the First Affiliated Hospital of Kunming Medical University.The lesions were biopsied(puncture,minimally invasive,open surgery)and histopathological diagnosis was clear.A total of 3579 patients had pathology 3590 specimens.After screening,it met the research standards and grouping requirements,with a total of 135 patients and 144 pathological specimens.A retrospective analysis of 92 patients with a total of 99 groups of pathological specimens.Ultrasound BI-RADS was classified as 4b,4c,and 5 types,the results of the medical examination were benign lesions.A retrospective analysis of 43 patients with a total of 45 groups of pathological specimens.Ultrasound BI-RADS was classified as 2,3 and 4a types,the results of the examination are high-risk lesions:borderline lesions,atypical ductal hyperplasia(ADH)and atypical lobular hyperplasia(ALH)and malignant lesions.This paper analyzed the performance of the sonogram and combine histopathological results,summarize the results of the ultrasound BI-RADS classification of breast lesions and the diagnosis of pathological diagnosis.The pathological types and sonographic manifestations of inconsistent lesions of benign and malignant,summarize the relationship between the sonogram of breast lesions and the pathological manifestations.Discuss how to diagnose the same disease with the same disease in the ultrasound image of breast.Results1.The results of the examination were benign lesions.Ultrasound BI-RADS was classified as 44.8b(84/99),4c 11.11%(11/99),and 5 4.04%(4/99).2.The results of the examination were high-risk lesions(border lesions,atypical ductal hyperplasia,atypical lobular hyperplasia)and malignant lesions.Ultrasound BI-RADS was classified as category 2 0.00%(0/45),category 3 15.56%(7/45),4a category 84.44%(38/45).3.The shape,edge,border,internal echo,and rear echo ultrasound features of the lesion have statistically significant differences in the ultrasound BI-RADS classification(P<0.05).The shape,edge,border,internal echo,and rear echo ultrasound features of the lesion can be considered has an impact on the BI-RADS classification.4.The growth direction of the lesion and the characteristics of calcified ultrasound were not statistically different in the ultrasonic BI-RADS classification(P>0.05).It cannot be considered that the growth direction of the lesion and the characteristics of calcified ultrasound had an effect on the BI-RADS classification.Conclusions1.BI-RADS classification standardizes and standardizes the ultrasound examination of breast lesions.Compared with the traditional ultrasound diagnosis of breast lesions without discrimination,it has a higher accuracy.2.Some benign breast lesions can show benign changes in malignant lesions on ultrasound,and most of them are diagnosed as BI-RADS 4b,4c,5 types.3.Some high-risk lesions and malignant breast lesions can show benign lesion changes on ultrasound,and most of them are diagnosed as BI-RADS category 3 and 4a.4.The changes of ultrasound sonograms of breast lesions are closely related to histopathological types and manifestations.
