| Objective:To explore the predictive value of ABCD2 score,Cystatin C and ABCD2 score combined with cystatin C in the risk of cerebral infarction in patients with early transient ischemic attack.Methods:From October 2019 to December 2020,hospitalized patients with TIA were selected in the Department of Neurology of our hospital.The routine serum samples and serum cystatin C test were completed within 24 hours after admission,and the acbd2 score was used to evaluate the latest TIA attack.All patients were treated with aspirin,clopidogrel combined with antiplatelet and statins,with the seventh day as the end The patients were divided into TIA group(n = 105)and cerebral infarction group(n = 35).The difference of cystatin C level between the two groups was compared.The predictive value of ABCD2 score,Cystatin C and their combination on early stroke risk of TIA patients was evaluated by receiver operating characteristic curve(ROC curve).Results:Compared with TIA group,the level of cystatin C in cerebral infarction group was significantly higher,and the difference was statistically significant(P<0.05).The AUC of ABCD2 score in predicting cerebral infarction within 7 days was 0.703,the best critical value was 4 points,the sensitivity was 85.7%,the specificity was 50.5%,and the 95%confidence interval was 0.607 ~ 0.799(P<0.05);The AUC of cystatin C in predicting cerebral infarction within 7 days was 0.797,the best critical value was 0.925mg/l,the sensitivity was 70.1%,the specificity was 73.3%,and the 95% confidence interval was0.703 ~ 0.890(P < 0.05);The AUC was 0.842,the sensitivity was 79.3%,the specificity was 81.0%,and the 95% confidence interval was 0.759 ~ 0.925(P<0.05).The AUC of the combination of the two was compared with that of ABCD2 score,z=2.179,P =0.014<0.05,which indicated that the combination of the two had better prediction effect.Conclusion:As a serum biomarker,Cystatin C can predict the risk of cerebral infarction in patients with TIA.The predictive value of ABCD2 score combined with cystatin C is better than that of the two alone. |
