Objective:To investigate the effect of home-based immediate outpatient period cardiac exercise rehabilitation in low exercise risk patients with coronary heart disease after percutaneous coronary intervention,and make comparison with centre-based rehabilitation.Methods:Select 108 low exercise risk patients with coronary heart disease after PCI who were hospitalized in the Department of Cardiology of First Hospital of Shanxi Medical University from September 2019 to September 2019.According to personal willingness,built up 3 groups : the conventional treatment group(CTG)、 the centre-based rehabilitation intervention group(CIG),home-based rehabilitation intervention group(HIG),each group contains 36 cases.The CTG patients were treated with routine treatment including health education、medication prescription and routine discharge guidance.While CIG and HIG patients will take exercise prescriptions and regularly exercise in the hospital rehabilitation center and home respectively,at least 25 times training should be done for each patient.The program last for 3 months and the difference of related indicators between before and after the program will be investigated.Results:During the follow up,3 cases were lost,4 cases did not complete the program,and 101 patients were finally included in this research(35 cases in CTG,32 cases in CIG,34 cases in HIG).Comparison between after and before treatment : BMI CIG23.48±1.48>25.52±2.69,P<0.001,HIG23.55±1.91>25.58±3.18,P<0.001,CIG:HIG P>0.05,all lower than CTG(P=0.022);BMI Compliance rate:CIG56.3%>31.3%,P=0.220,the rest comparison P>0.05;Waist CTG91.21±9.10>91.43±9.10,P=0.007,CIG84.97±7.79>91.06±8.21,P<0.001,HIG85.44±8.35>90.44±9.95,P<0.001,CIG and HIG are lower than CTG P=0.004,the rest comparison of waist and waist compliance rate P>0.05;Smoking rate CIG9.4%<46.9%,P=0.004,HIG11.8%<41.2%,P=0.004,CIG:HIG P>0.05,all lower than CTG(P=0.006);drinking rate CIG6.3%<34.4%,P=0.005 CIG: CTG P>0.05,HIG:CTG P<0.05;Blood pressure compliance rate CIG87.5%>50.0%,P=0.001,HIG82.4%>52.9%,P=0.010,CIG:HIG P>0.05,all lower than CTG P=0.008;TC CTG3.82±1.20<4.00±1.26,CIG2.97±0.49<3.74±1.07,HIG3.09±0.55<3.90±1.14,all P<0.001,CIG:HIG P>0.05,all lower than CTG P<0.001;TC compliance rate CIG68.8%>18.8%,P<0.001,HIG61.8%>29.4%,P=0.007,CIG:HIG P>0.05,all higher than CTG P=0.002;TG CTG1.51±0.65<1.52±0.66,CIG1.34±0.35<1.54±0.57,HIG1.36±0.33<1.57±0.52,all P<0.001,CTG:CIG:HIG P>0.05;TG compliance rate CIG93.8%>68.8%,P=0.010,HIG91.2%>58.8,P<0.001,CIG:HIG P>0.05,all higher than CTG P=0.018;LDL-CCTG2.38±0.87<2.47±0.92,CIG1.74±0.36<2.32±0.81,HIG1.80±0.43<2.45±0.85,all P<0.001;LDL-C compliance rate CIG68.8%>21.9%,HIG64.7%>20.6%,all P<0.001,CIG:HIG P>0.05,all higher than CTG P=0.003;Resting heart rate CTG73.94±10.85<75.00±10.65,CIG68.69±7.71<72.03±7.80,HIG68.44±6.57<71.94±6.59,all P<0.001,CIG:HIG P>0.05,all higher than CTG P=0.014;LVEF CTG60.43±5.20>58.43±5.23,CIG66.41±6.48>61.00±6.37,HIG65.88±4.79>59.47±4.19,all P<0.001,CIG:HIG P>0.05,all lower than CTGP<0.001。there is no significant difference in comparisons related to HDL-C、HDL-C compliance rate and blood glucose(all P>0.005)。Conclusion:Low exercise risk patients with coronary heart disease after PCI employ home-based immediate outpatient period cardiac exercise rehabilitation could lead to good results in controlling body shape,quitting smoking and alcohol,controlling hypertension 、 blood lipids,and increasing LVEF,decreasing resting heart rate.Home-based rehabilitation could achieve similar results as center-based rehabilitation.
