Objective:To analyze the characteristics of blood lipid levels in chronic constipation and non-chronic constipation,and the distribution of confounding factors was controlled based on propensity score matching(PSM).Methods:From April 2019 to October 2020,a total of 350 patients with chronic constipation in the first Hospital of Shanxi Medical University were enrolled,and a total of 318 patients with non-chronic constipation were collected in the same period.The diagnosis of chronic constipation draws lessons from the symptoms and course of functional constipation described in Rome IV standard.Chronic constipation group was divided into the mild group,the moderate group and the severe group according to Wexner scoring system.Collect general demographic data,concomitant diseases(including hypertension,diabetes,carotid atherosclerosis),blood lipid indexes [including: total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C)] and daily living habits(smoking,drinking,sleep quality,diet and exercise habits).In the SPSS26.0 software,PSM was used for controlling the general demographic data,concomitant diseases(hypertension,diabetes),living habits and other confounding factors for data statistics and analysis.Results:1.After PSM,151 cases of chronic constipation and 151 cases of non-chronic constipation were obtained.There were 79 males and 72 females with an average age of(63.07±14.93)years in chronic constipation group and 76 males and 75 females with an average age of(63.05±16.45)years in non-chronic constipation group.There was no significant difference in the general data between the two groups(P > 0.05).2.According to the stratification standard of blood lipid level,the percentage of marginal elevated and elevated in TC,TG and LDL-C levels and the percentage of decrease in HDL-C level in chronic constipation group were higher than that in non-chronic constipation group(P≤0.01).3.According to the clinical classification of dyslipidemia,the rates of hypercholesterolemia,hypertriglyceridemia,mixed hyperlipidemia and low high-density lipoprotein cholesterol levels in chronic constipation group were 37.7%,39.7%,23.3% and 26.5%,and those in non-chronic constipation group were 11.9%,20.5%,6% and 14.6%.The proportion of dyslipidemia in chronic constipation group was higher than that in non-chronic constipation group(P≤0.01).4.In terms of blood lipid levels,the levels of TC,TG and LDL-C in chronic constipation group were higher than those in non-chronic constipation group(P<0.05).LDL-C and TC changed significantly with the severity of chronic constipation.HDL-C level in chronic constipation group was lower than that in non-chronic constipation group(P=0.02).5.The proportion of carotid atherosclerosis in the chronic constipation group was higher than that in the non-chronic constipation group(36.4% vs.23.8%,P=0.017).Conclusion:The prevalence rate of dyslipidemia in chronic constipation is higher,and the overall levels of TC,TG and LDL-C are higher than those in non-chronic constipation,in which the changes of TC and LDL-C are related to the severity of chronic constipation,and the prevalence rate of carotid arteriosclerosis in chronic constipation is higher.Therefore,attention should be paid to the blood lipid level of chronic constipation and clinical management.
