Study On The Content Of Nurse Prescription In Dermatology And Burns Orthopedics

Objective:The purpose of this study is to explore the prescription drugs and prescription forms that can be prescribed by nurses who meet the qualifications of nurses in the Department of Dermatology and Burn plastic surgery of Grade 3A Hospital in China.in order to provide a certain reference basis for the formulation and pilot implementation of nurses’ prescription right health policy in our country in the future.Methods:In this study,literature investigation,semi-structured interview and Delphi method were used to study the prescription contents and forms of nurses’ skin system and surgical drug prescription right in China.1.Literature investigationUse nurses,prescription rights and dermatology as keywords to search foreign dermatology and burn plastic medicine databases such as Pub Med,CINAHL,Scopus,etc.,and look up the prescriptions of nurses in the United States(Ohio),Britain,Australia,the United Kingdom and South Africa on the foreign official website.Translation of the above prescriptions is concentrated in dermatology and burn plastic medicine,and referring to the clinical drug manual(fifth edition)published by Shanghai Science and Technology Press of my country to develop the initial questionnaire.2.Semi-structured interviewBy consulting the literature and group discussion to form a semi-structured interview outline,4 medical specialists and 4 nursing specialists in dermatology,4medical specialists and 4 nursing specialists in burn orthopedic surgery were selected to conduct semi-structured interviews on the initial questionnaire,and the consultation questionnaire was revised and sorted out according to the expert advice.finally,the first round of expert consultation questionnaire on dermatology and surgical drug prescription right of nurses was formed.3.Delphi methodTo select medical experts and nursing experts who meet the inclusion criteria among the experts who agree to grant nurses the right to prescribe,the inclusion criteria of experts are as follows:(1)associate professor and above;(2)engaged in this major for10 years or more;(3)bachelor degree or above;Strict and realistic attitude,voluntary participation.The first three items meet two items.Therefore,the consultation questionnaire on drug prescription right of dermatology nurses selected 36 dermatologists from the third-class hospitals of Shanxi,Fujian,Beijing and other 12 provinces /municipalities directly under the Central Government.The questionnaire on drug prescription right of nurses in burn plastic department selected 36 burn plastic surgery experts from 10 provinces / municipalities directly under the Central Government,including Fujian,Hubei and Shanxi,for two rounds of expert argumentation.Results:1.Contents and forms of prescription for nurses in dermatology departmentThe positive coefficients of the two rounds of experts are 88.89% and 100.00%respectively,and the expert authority coefficients are both 0.81.It is determined that there are 13 categories and 63 kinds of dermatological drugs that can be prescribed by nurses in the department of dermatology of Grade 3A Hospital in China.Among them,there are 31 kinds of agreed or extended prescriptions,17 kinds of agreed prescriptions,10 kinds of extended prescriptions,3 kinds of extended or adjusted prescriptions,1 kind of independent prescriptions and 1 kind of independent or agreed prescriptions.The specific drugs in each prescription form are: 1 the drugs in the agreed or extended prescription form are: silicone oil,tannic acid,zinc sulfate,dyclonine,bacitracin,colistin,clindamycin,mercuric chloride,hydrocortisone,momethasone furoate,coal tar,fish fat,p-aminobenzoic acid,titanium dioxide,ketoprofen,ibuprofen,compound clotrimazole,compound halomethasone,minoxidil,vitamin B1,propylthiamine,furan thiamine,furan thiamine.Long-acting riboflavin,nicotinic acid,nicotinamide,calcium pantothenate,vitamin B4,rutin,Alfadizol,Vioxen,multidimensional elemental tablets.2 drugs in the form of agreed prescription: zinc oxide,potassium permanganate,starch,furacilin,menthol,mupirocin,fusidic acid,urea,allantoin,polysulfonate mucopolysaccharide cream,vitamin B2,vitamin C,vitamin A,vitamin D,Vikangfu,compound vitamin B,calcium.3 drugs in extended prescription form: bromobenzylamine,camphor,gentamicin,norfloxacin,hydrocortisone butyrate,furfural distillate,compound econazole nitrate,capotriol,tacasil,hydrovitamol;4 drugs in extended / adjusted prescription form:phenyl salicylate,etofenac sodium ester,compound triamcinolone acetonide;5 drugs in independent prescription form: iodophor;6 drugs in independent / agreed prescription form: calamine.2.Contents and forms of prescriptions for nurses in the department of burn plastic surgeryThe positive coefficients of the two rounds of experts are 88.89% and 100.00%respectively,and the expert authority coefficients are 0.82 and 0.81 respectively.Finally,it was determined that the surgical drugs that could be prescribed by nurses in the Department of Burn plastic surgery in Grade 3A Hospital were 5 categories and 26kinds of drugs.among them,10 kinds of drugs tend to be agreed prescriptions,9kinds of drugs tend to agree / extend prescriptions,4 kinds of drugs tend to be independent or agreed prescriptions,and 1 kind of drugs tend to be independent,prolonged and independent / extended prescriptions respectively.The specific drugs in each prescription form are as follows: 1)10 drugs in the agreed prescription form:ethylene oxide,peracetic acid,castor oil acetate,silver sulfadiazine,Kangruibao,Mabel moist burn ointment,new dermatology silicone gel,liquid paraffin,mupirocin ointment.2)9 drugs that tend to agree or prolong prescription: iodine tincture,potassium permanganate,hydrogen peroxide solution,furacilin,oral washing intestine powder,Yuchuang blue embers,etoposide cream,fish gypsum ointment,absorbent gelatin sponge;3)4 drugs that tend to be independent or agreed prescription: benzalkonium bromide,ethanol,chlorhexidine acetate,glutaraldehyde;4)1 drug that tends to be independently prescribed: povidone iodine.5)1 drug that tends to prolong the prescription: ibuprofen cream;6)1 drug that tends to be independent or prolonged prescription: soft soap solution Conclusion:In this study,through two rounds of expert letters,it is concluded that 13 categories of 63 kinds of skin system drugs and 26 kinds of surgical drugs can be prescribed by nurses in dermatology department of third-class and first-class hospitals in China.The main forms of prescription are agreement or extension prescription and agreement prescription.The two rounds of experts have high enthusiasm and authority,so this study is reliable and scientific.
