| ObjectiveVitiligo is a damaging disease that can occur in any part of the face,neck,trunk,limbs,etc.It is relatively difficult to treat.For stable vitiligo,which is not effective in traditional medicine and phototherapy,autologous melanocyte transplantation is the most effective method.One of the effective treatments.Autologous cultured melanocyte transplantation is mature,but there are still patients with poor transplantation efficacy.Past studies have shown that clinical classification,skin lesion location,disease course,abnormal thyroid function,etc.can affect the efficacy of transplantation;newer studies have shown that peripheral Abnormal levels of blood chemokines and cytokines are related to the efficacy of transplantation.The research team verified that the levels of CXCL9,CXCL10,and CXCL11 are related to vitiligo activity,and the levels of these three chemokines decrease after transplantation.However,there are few reports on whether the levels of these three groups of chemokines affect the efficacy of transplantation.Therefore,This study aims to explore the relationship between the efficacy of autologous cultured melanocyte transplantation and the levels of CXCL9,CXCL10,and CXCL11,and to analyze the relationship between the efficacy of transplantation and age,gender,course of disease,type,and thyroid function.MethodsFrom January 2019 to July 2020,patients with vitiligo who were treated at the Department of Dermatology,People’s Hospital of Xinjiang Uygur Autonomous Region,all patients completed autologous cultured melanocyte transplantation in the undergraduate room,and were finally included in accordance with the diagnostic criteria,inclusion criteria and exclusion criteria 15 cases in the recolor ≥50% group,13 cases in the recolor <50% group,28 patients in total,and 28 cases in the control group.All patients in the experimental group were drawn 5ml of peripheral venous blood into the red blood collection tube on an empty stomach before transplantation,and recolor For the ≥50% group,5ml of peripheral venous blood should be collected 6 months after transplantation,and the levels of CXCL9,CXCL10,and CXCL11 in the peripheral blood were determined by ELISA.At the same time,we collected clinical data and laboratory test indicators of transplant patients,using SPSS25.0 The software performs statistical analysis.Result1.A total of 83 skin grafts,including 18 on the face(21.69%),8 on the neck(9.64%),23 on the trunk(33.74%),14 on the limbs(16.87%),and 5 on the joints(6.02%)),15pieces(18.07%)of the extremities of the hands and feet,of which the trunk area has the most skin grafts,and the joint area has the least skin grafts.2.Among the patients with abnormal thyroid function,the number of patients with abnormal TSH value was significantly higher than that of other T3/T4,TGAb,and TPO values,which were 6(26.09%),12(52.17%),3(13.04%)and 2(8.70%).3.The levels of CXCL9,CXCL10,and CXCL11 before transplantation in the same successful patient were higher than those after transplantation,and there were statistical differences,respectively(Z=-2.8,P=0.005;Z=-2.8,P=0.005;Z=-2.366,P=0.018).4.Serum CXCL9,CXCL10,and CXCL11 levels in patients with vitiligo are: recolor<50% group> recolor ≥50% group> normal control group,the overall levels of chemokines in the three groups are statistically different,(F=8.84,P=0.012;F=26.26,P=0.000;F=7.35,P=0.025).5.Serum CXCL10 levels in patients with segmental vitiligo are higher than those of non-segmental types,and there is a statistical difference between the two(z=-1.97,P=0.049);serum CXCL9,CXCL10,and CXCL11 levels in patients with vitiligo are in gender and course There was no statistical difference between them(P>0.05);the serum CXCL10 level of patients with abnormal thyroid function and vitiligo was higher than the normal level of thyroid function,and there was a statistical difference between the two(z=-2.60,P=0.009).6.Age,gender,course of disease and classification were not statistically significant between the recolor ≥50% group and the recolor <50% group(P>0.05).Whether accompanied by thyroid dysfunction was statistically significant between the two groups(2=12.253,P=0.000).7.Univariate Logistic regression found that CXCL10 and thyroid function are risk factors that affect the efficacy of transplantation,respectively(OR=7.8,95%CI 为6.242-9.740,P=0.001;OR=3.3,95%CI 为 3.182-12.157,P=0.003).conclusion1.The level of CXCL9,CXCL10,CXCL11 may be one of the important indicators that affect the efficacy of transplantation,especially the increase of CXCL10 level is related to the poor efficacy of vitiligo transplantation.2.Different age,sex,course of disease and classification have no obvious influence on the effect of transplantation.3.Thyroid function is one of the risk factors that affect the efficacy of transplantation.Abnormal thyroid function may lead to poor transplantation efficacy. |
