| Object:To evaluate the therapeutic effect of autologous melanocyte culture and transplantation in the treatment of vitiligo,and to explore the difference of transplantation efficacy among different clinical types,different skin graft sites and different stable time of vitiligo patients.Methods: A total of 132 white spot lesions were selected from 59 patients with stable vitiligo who met the inclusion and exclusion criteria,including 45 lesions in 20 patients with generalized vitiligo,51 lesions in 24 patients with localized vitiligo and 36 lesions in 15 patients with segmental vitiligo,25 lesions in the face,35 lesions in the trunk,39 lesions in the limbs and 33 lesions in the hands and feet.All patients were treated with autologous melanocyte culture and transplantation.At 6months after transplantation,clinical follow-up was conducted to observe the efficacy,side effects and complications,and to evaluate the rate of leukoplakia repetition and the occurrence of side effects(scar formation,infection and enlargement of the original leukoplakia area).Results:The therapeutic effect of autologous melanocyte culture and transplantation in the treatment of vitiligo is worth affirming.The clinical classification and transplantation site of vitiligo can affect the therapeutic effect of autologous melanocyte transplantation.However,the duration of the patient’s disease has no significant effect on the treatment.A total of 132 lesions of leukoplakia were found in 59 patients,69 lesions(52.27%)were duplicated more than 90%,36 lesions(7.27%)were duplicated more than 50-90%,and the distribution of duplicate color was abnormal.There was no obvious scar formation or other adverse reactions.The total effective rate was 79.54%.The transplantation effect of segmental vitiligo patients was similar to that of localized vitiligo patients.For general hair type,skin graft in the face and neck region had the best effect(88.00% effective rate),while hand and foot region had the worst effect(63.6%).Conclusion: Autologous cultured melanocyte transplantation is effective in the treatment of stable vitiligo.There are significant differences between different clinical types of vitiligo(P=0.004),and between different transplantation sites(P=0.001).There is no significant difference between different stages of vitiligo(P=0.488). |
