| Background and Objectives: Breasts are an important organ for women and a source of confidence.Therefore,more and more women pay attention to breast augmentation.There are many materials for breast augmentation,among which fatty materials are easy to obtain,cheap and have good histocompatibility,and are regarded as ideal fillers.Autologous fat augmentation mammaplasty refers to the use of negative pressure technology to extract the fat from the thighs,abdomen and buttocks.After special treatment,the fat is injected into the chest with a thin blunt needle in accordance with the principle of multi-level,multi-tunnel,multi-plane and a small amount at a time,so as to achieve the effect of breast fullness.However,autologous fat transplantation has low survival rate,easy absorption,and the risk of fat calcification,necrosis,cystic fibrosis and pseudocyst,which makes it difficult for patients to accept it [1].Fat survival requires higher blood supply,and the early establishment of new blood vessels is of great significance to improve the survival rate of fat.Platelet-rich fibrin(PRF)[2]is a second-generation platelet concentrate with three-dimensional structure and rich growth factors,which provides conditions for endothelial cell migration and proliferation and promotes angiogenesis.This study investigated whether PRF-assisted autologous fat augmentation had higher fat survival than fat augmentation alone Materials and Methods: In this study,36 women who underwent autologous fat transplantation for breast augmentation in the Department of Plastic Surgery,The Second Affiliated Hospital of Anhui Medical University from July 2018 to June 2020were selected.Among them,15 were breast atrophy accompanied by mild ptosis after breastfeeding,5 were bilateral breast dysplasia,and 16 were unilateral breast dysplasia,a total of 56 breasts,aged 18-48 years.According to random grouping method divided the patients into two groups: observation group set to A set of auxiliary autologous fat transplantation group(PRF),average age(31 + 7),the control setting of group B(simple autologous fat transplantation group),average age 29(+ 7),two groups of age,there was no statistically significant difference(P > 0.05),two groups of patients with preoperative nipple roots to the plane of the anterior chest wall vertical distance difference has no statistical significance(P > 0.05),preoperative conventional breast ultrasound,clear whether there is A nodule,and so on and so forth;All subjects signed the informed consent before surgery.Observe the subjects after 2 weeks,1 month,3months,6 months nipple uplift value(upright condition nipples roots to the plane of the anterior chest wall vertical distance and the distance difference between preoperative),satisfaction with the operation,postoperative complications rate,etc.,on the basis of the above data chart,use SPSS statistical methods to determine whether a difference have statistical significance;Results: After the treatment,the symptoms of breast ptosis or dysplasia in all patients were significantly improved compared with those before the treatment,and the patients themselves were relatively satisfied.Before treatment,there was no significant difference in age and nipple elevation between groups,and the data were comparable.After two weeks of treatment,breast lumps in group A and group B were14.24±2.48 and 14.22±2.51,respectively,and there was no significant difference between groups.After 1 month of treatment,the breast lumps of group A and group B were 12.56±2.07 and 11.50±1.80,respectively,and the lumps of group A were higher than that of group B,with statistical significance(P<0.05).3 months after treatment,the breast lumps in group A and group B were 11.02±1.73 and 9.08±1.05,respectively,P <0.01,and the difference was statistically significant.At 6 months after treatment,breast lumps in group A and group B were 10.93±1.73 and 8.54±0.62,respectively,the difference was significant and statistically significant.The satisfaction of the observation group was 88.89%,and the satisfaction of the control group was72.22%.The rate of fat sclerosis,liquefaction and other complications in the observation group was 5.56%,while the rate of complications in the control group was16.67%.No serious complication or adverse reactions occurred in all subjects before and after the whole treatment process.Conclusions: Platelet-rich fibrin(PRF)assisted autologous fat augmentation can improve the survival rate of fat transplantation,reduce postoperative complications and improve patient satisfaction.In addition,fat is easy to obtain,simple to operate,low cost,no rejection reaction compared with prosthetic breast augmentation,more easily accepted by patients,worthy of further clinical promotion,but the specific mechanism of its therapeutic effect still needs to be further explored and studied... |
