| Objective: To compare the efficacy and safety of 2940 nm erbium yttrium aluminum garnet(Er: YAG)ablative fractional laser(AFL)with β-glucan and 2940 nm erbium yttrium aluminum garnet ablative fractional laser alone in the treatment of striae gravidarum(SG)through self-control study.Methods: Twenty volunteers(skin type Fitzpatrick III-IV)with striae gravidarum on the abdomen were enrolled.Abdomen area was divided into A B side by the midline.The side A only received 2940-nm Er: YAG AFL therapy.Treatment parameters: exfoliation150-200 um,0-1 level coagulation,coverage rate 11%.Once every time.The side B was treated with the same parameters,except that β-glucan was added.Both were treated 3times at 4-week intervals.Treatment effect evaluation: Evaluated the effects of the two treatment methods through doctor’s subjective evaluation,subjective evaluation by subjects,high-frequency ultrasound detection,reflectance confocal microscopy(RCM),etc,and record the occurrence of adverse reactions after treatment: such as pain score,duration of erythema and edema,days of crusting,pigmentation,etc.Results: A total of 20 patients completed this experimental study.Doctor’s subjective evaluation: Both treatment sides reported significant clinical improvements of SG,significantly higher degree of clinical improvements were observed in side B。The results of statistical analysis showed that the clinical efficacy difference between the two sides was statistically significant(P=0.001,P<0.05).Subjective evaluation of the subjects: the patient’s satisfaction with the improvement of stretch marks.The satisfaction rate was 60%in side A,75% in side B.The subjects were more satisfied with the B side,There was a statistically significant difference in patient satisfaction between the two treatment methods(P=0.003,P<0.05).The thickness of A side was detected thicking by high-frequency color doppler ultrasound after three times treatments,and the difference was statistically significant(P<0.05).The skin thickness of B side after three times treatments was greater than that before treatment,and the difference was statistically significant(P<0.05).The skin thickness of B side stria gravidarum before and after treatment was greater than that of A side stria gravidarum before and after treatment,the difference was statistically significant(P=0.035,P<0.05).The thickness of striae gravidarum was detected thicking by high-frequency color doppler ultrasound after three times treatments on both sides.According to statistical analysis,the difference before and after treatment was statistically significant(P<0.05).RCM results showed that the collagen content of striae distensae increased after treatment.There was significant difference in the pain score evaluation(P=0.002,P<0.05),as the score was higher on side A than that of the side B.After treatments,the regression time of erythema edema in side B was shorter than that in side A,and the difference between the two groups was statistically significant(P<0.05).After treatment,the time of decrustation on the B side was shorter,and the difference between the two sides was statistically significant(P=0.002,P< 0.05).Pigmentation occurred in 2 cases(10%)on both sides after treatment,subsided before follow-up,There was no significant difference between the two sides.Exudate occurred in 5 patients(25%)on both sides,There was no significant difference between the two sides.Conclusion: Both the 2940-nm Er: YAG AFL and the 2940-nm Er: YAG AFL combined with β-glucan can improve striae gravidarum effectively and safely.and the efficacy and safety of combined treatment side is higher than that of single treatment side. |
