BackgroundWith the continuous improvement of perinatal medical technology,the level of treatment of neonatal respiratory distress syndrome(NRDS)has been significantly improved,but it is still one of the critical diseases affecting the quality of life of neonates.It is believed that the incidence of NRDS is related to preterm birth,gestational diabetes mellitus(GDM),cesarean section and so on.The incidence of GDM in China is higher than the world average,which is one of the important factors affecting the occurrence of NRDS.It is believed that the incidence of NRDS in controlled mothers and infants with GDM is no different from that of mothers and infants without GDM.However,whether the occurrence of NRDS is related to cord blood glucose and cord blood hormones under stress state is still to be studied,so as to provide some theoretical basis for the preventive health care of mothers and infants with GDM.ObjectiveBy comparing the general data of mothers and newborns with GDM,the levels of blood glucose,glycated hemoglobin,cortisol and thyroid hormone in cord blood of mothers and infants with GDM were detected,and the risk factors of NRDS in mothers and infants with GDM and the correlation between blood glucose,cortisol and thyroid hormone in cord blood and NRDS were discussed.MethodsA total of 118 cases of GDM pareras who delivered in obstetrics department of the First Affiliated Hospital of Xinxiang Medical University from November 2019 to February 2021 were selected.According to the inclusion criteria and exclusion criteria,29 cases of GDM mothers and children with NRDS who received treatment in the neonatal care unit of the First Affiliated Hospital of Xinxiang Medical University were finally selected as the study group.49 mothers and children with GDM without NRDS were randomly selected as the control group.Mothers in both groups were diagnosed with gestational diabetes by oral glucose tolerance test.The general data of mothers and newborns in the two groups were recorded,and the levels of blood glucose,HbA1c,cortisol(COR),free triiodothyronine(FT3),free thyroxine(FT4)and thyroid stimulating hormone(TSH)in the umbilical cord blood of newborns in the two groups were detected for correlation analysis.Results1.There was significant difference in birth weight and gestational age between the two groups(P<0.05).There was no significant difference in gender between the two groups(P>0.05).Comparison of general data of pregnant women between the two groups showed statistically significant differences in pregnancy weight,adverse pregnancy history,hypertension during pregnancy and prenatal hormone application(P<0.05).There were no significant differences in age,number of fetuses,delivery mode,premature rupture of membranes,hemoglobin A1c in umbilical cord blood and delivery asphyxia(P>0.05).2.Gestational hypertension,birth weight < 2.5kg and gestational age < 37 weeks were the risk factors for NRDS in mothers and infants with GDM(P<0.05).3.There was no significant difference in blood glucose and cortisol in umbilical cord blood between the study group and the control group(P>0.05).4.There was no significant difference in umbilical cord blood FT3 and FT4 between the study group and the control group(P>0.05).The neonatal cord blood TSH in the study group and the control group was higher than that in the study group,and the difference was statistically significant(P<0.05).5.There was no correlation between cord blood glucose and cord blood cortisol in the study group(P>0.05).There was no correlation between umbilical cord blood glucose and FT3(P>0.05).There was no correlation between umbilical cord blood glucose and FT4(P>0.05).There was no correlation between cord blood glucose and cord blood TSH(P>0.05).Conclusion1.Gestational hypertension,small gestational age and low birth weight are risk factors for NRDS in the newborn of mothers with GDM.2.NRDS is more likely to occur in neonates of GDM mothers with low TSH in the normal range of hemoglobin A1 c.3.There was no correlation between umbilical cord blood glucose,cortisol and thyroid hormone and the occurrence of NRDS.
