Objective:To explore the risk factors of recurrence of Henoch-Schonlein purpura(HSP)in adults through retrospective research,and to suggest early intervention by clinicians to reduce recurrence.Method:To retrospectively analyze the clinical data of adult HSP hospitalized patients who were first diagnosed from January 2015 to December 2019 in the Second Affiliated Hospital of Nanchang University.According to the follow-up results,the selected HSP patients were divided into recurrence group and non-recurrence group according to the follow-up results,and the age,gender,onset season,onset cause,first symptom,clinical classification,laboratory indices,and treatment medication were compared between the two groups.The statistical software SPSS 26.0 was used to process and analyze the two sets of data,in which the count data was turned into percentage,and chi square test was used to compare the percentage difference.Mean differences between the two groups was compared using the two independent samples t-test.Finally,the statistically significant data after univariate analysis were included in multi-factor Logistic regression analysis.Results:1.The data of this study included 106 adult HSP patients,including 54 males(50.9%)and 52 females(49.1%),the male-to-female ratio was 1.03:1.0,the minimum age was 18 years old,and the maximum age was 65 years old,the average onset age(35.09±13.66)years old,All of the 106 HSP patients had purpura(100%).there were a rash and purpura manifestation(100%),32 cases(30.2%)with joint symptoms,33 cases(31.1%)with abdominal symptoms,50 cases(47.2%)with kidney damage.2.A single factor analysis of risk factors related to the recurrence of Henoch-Schonlein purpura in adults: In clinical features,there were statistically significant differences between the two groups in infection,persistent rash,and renal damage(P<0.05),while gender,age,onset season,hypertension,joint symptoms,abdominal symptoms have nothing to do with recurrence;among the laboratory indicators,white blood cell(WBC)increase,mean platelet volume(MPV),urine protein positive,hematuria were significantly different between the two groups(P<0.05),and the absolute value of neutrophils count,lymphocytes count,eosinophil count,basophil count,platelet(PLT),Ig G,Ig A,Ig M,C3,C4,albumin(ALB)has nothing to do with recurrence;in terms of treatment drugs,no antihistamines,cortisol,immunosuppressants and other drugs have been found to affect the recurrence of HSP in adults.3.The results of Logistic multi-factors regression analysis showed: infection,persistent purpura,increase in average platelet volume,and positive urine protein are the independent risk factors of the recurrences of adult HSP(P<0.05).Conclusion:1.The recurrence rate of adult HSP is 25.5%,and the peak period of recurrence is within 6 months of onset,they need to be closely watched.2.Infection,persistent purpura,elevated MPV value,and high urinary protein are independent risk factors for adult HSP recurrence.Leukocytosis and microscopic hematuria may be related to recurrence.3.Age,sex,season of onset,high blood pressure,gastrointestinal symptoms,joint symptoms,neutrophil count,lymphocyte count,eosinophil count,basophilia count,PLT,Ig G,Ig A,Ig M,C3,C4,ALB levels,and treatment medication have nothing to do with the recurrence of HSP.4.Clinicians should carry out early intervention for high-risk patients,shorten the course of patients’ disease,and provide help for their long-term prognosis.
