Objective: To investigate the relationship between different states of medication adherence and quality of life and survival of patients in a single-center,randomized,retrospective cohort study of patients receiving primary adjuvant endocrine therapy. Methods: A total of 209 cases of breast cancer patients who received primary adjuvant endocrine therapy after surgery, radiotherapy and chemotherapy in the Affiliated Hospital of Qinghai University from January to December 2017 were collected and followed up for 3 years. The overall drug possession rate of 3 years was calculated, and the patients were divided into three groups with good adherence, medium adherence and poor adherence.Hereby, this study is divided into two parts. In the first part, the correlation analysis between adherence and quality of life was conducted to calculate the scores of the QLQ-C30 scale of enrolled patients(including 15 dimensions with scores ranging from 0 to 100)and explore the relationship between adherence and quality of life in 15 dimensions respectively. The second part preliminarily explored the survival analysis of adherence, and compared the 3-year disease-free survival rate of patients in the good, moderate and poor groups. Results: Part I,Correlation analysis of medication adherence and quality of lifeMedication adherence of adjuvant endocrine therapy was correlated with the five dimensions, which contain emotion dimension(rs=0.351,P =0.005),cognitive dimension(rs=0.328,P <0.01),social dimension(rs=0.415,P <0.01),overall health status(rs=0.408,P <0.01),pain(rs=-0.310,P <0.01),economic difficulty(rs=-0.446,P <0.01)in the QLQ-C30.Part Ⅱ,Preliminary study on survival analysis of adherenceGood adherence,Poor adherence and moderate adherence, these groups of patients 1 year DFS rate were 100.00%,100.00%,100.00%,2 years DFS rate were96.77%,100.00% and 100.00%,3 years DFS rate were 95.16%,97.53% and96.97%.Respectively,DFS rate comparing two of three groups were no differences(P =0.721).Among the subgroups with different menstrual status,Her-2 status and stage,3-year DFS rate among these groups of whom with good, medium and poor adherence were no difference. Conclusion: In this study, for breast cancer patients undergoing adjuvant endocrine therapy, the status of medication compliance was correlated with the emotional, general health, cognitive, social, pain, and economic difficulties dimensions of quality of life. There was no significant difference in short-term prognosis between different states of adherence.
