ObjectiveIschemic stroke is one of the main death and disability as chronic noncommunicable diseases,irreversible neuronal cell death will continue to cause reversible secondary damage on the brain,inflammatory cytokine cascade plays an important role in this process,however,related research is not comprehensive enough.The purpose of this paper was to systematically analyze the correlation between inflammatory cytokines and the occurrence,development,and prognosis of IS,and to find biomarkers that can be used to predict the occurrence of IS.MethodsA total of 75 patients with AIS(acute ischemic stroke)and 32 patients with TIA(transient ischemic attack)were collected.At the same time,72 healthy control samples matched with the AIS group(control 1)and 32 healthy control samples matched with the TIA group(control 2)were collected according to age and gender matching principles.In this study,bio-plex ProTM Human assay was used to systematically analyze the changes of serum concentrations of 15 cytokines in all populations,and established a model for predicting the occurrence of AIS disease,and tested its reliability.Secondly,we assessed the correlation between serum cytokine levels and clinical indicators of stroke,such as NIHSS score and infarct size.The correlation between serum cytokine levels and recurrence in patients with two follow-up visits(3 months and 6 months after discharge)was also analyzed.ResultsThe serum levels of IL-1β,IL-4,IL-6,IL-10,IFN-g,TNF-α,IL-21,IL-22,IL-23,IL-31,IL-17F,IL-33,and s CD40L were significantly higher in patients with acute ischemic stroke than in healthy control group 1.After adjusting for age,sex,BMI,hypertension,diabetes,coronary artery disease,TC,smoking,alcohol consumption and other intervention factors,IFN-g(p<0.001),TNF-(p<0.001),IL-21(p<0.001),and IL-22(p=0.004)were included in the prediction model of IS disease occurrence.Then,the accuracy of the above model was tested in TIA and paired control group 2.The results showed that the sensitivity and specificity of IL-21 for the prediction of IS was 0.938 and 0.531(AUC=0.777,P<0.001),which could be used as a candidate biomarker for the diagnosis and prognosis of IS.ConclusionIn summary,a comprehensive and systematic analysis of cytokines associated with the diagnosis of ischemic stroke,disease severity(e.g.,infarct size,NIHSS score),and treatment outcomes(whether or not hospitalized,)was performed,and some reliable results were obtained.This study further confirmed that the plasma levels of various cytokines in patients with ischemic stroke had significant changes,which had potential clinical application value in the clinical diagnosis,severity judgment and prognosis judgment of stroke and remained to be further explored.Our study gives some reliable results,but more samples are needed to verify our conclusions and explore new conclusions.
