Study On Clinical Features And Postoperative Recovery Of Patients With Early-onset Colorectal Cancer

Abstract/Summary:
PURPOSE: To explore the clinical features,postoperative recovery and factors affecting postoperative recovery in patients with early-onset colorectal cancer(EOCRC).Methods: We collected and analyzed the clinical data of 85 patients with earlyonset colorectal cancer who were hospitalized in Ruijin Hospital,Shanghai Jiaotong University School of Medicine from June 1,2010 to June 1,2019.37 patients who underwent radical tumor surgery from January 1,2013 to December 31,2018 were followed by phone calls and given two scales of gastrointestinal quality of life index(GIQLI)and personal financial health status(PFW).Results: Of the 85 patients,12(14.1%)patients had a history of malignant tumors of the family’s digestive tract,and 82(96.5%)patients were diagnosed by gastrointestinal symptoms.There were 43 patients(50.6%)with rectal cancer,43 patients with stage III(50.6%),and 15 patients with stage IV(17.6%).There were 12 cases(19.7%)of poorly differentiated adenocarcinoma,11 cases(13.6%)of mucinous adenocarcinoma,and 5 cases(6.2%)of signet ring cell carcinoma.There were 8 cases(21.6%)of MSI-H patients,including6 cases(75%)in the right colon and 1 case(12.5%)in the left colon and rectum.Of the 37 follow-up patients,27(73%)were able to return to their preoperative working state.The average score of GIQLI score is 80%,and the average score of PFW score is 54%.Gender(P = 0.018)and tumor location(P = 0.008)all have an effect on postoperative GIQLI in patients with early-onset colorectal cancer.Effects of intestinal symptoms(P = 0.034),physical condition(P =0.040),and social function(P = 0.015).Chemotherapy has a significant impact on patients’ recent social function(P <0.001).Traditional Chinese medicine(TCM)treatment has an impact on the patient’s recent physical condition(P= 0.021).Conclusion: Patients with EOCRC have a high proportion of family history of gastrointestinal tumors,mid-advanced tumors,poorly differentiated tumors,signet ring cell carcinoma,and MSI-H.Most patients can return to normal working conditions after operation,and the quality of life of the gastrointestinal tract can return to a good level after surgery.Patients have greater financial pressure in the near future.Male patients have fewer gastrointestinal symptoms than female patients after surgery.Patients with colon cancer have fewer gastrointestinal symptoms and better physical and social functions than those with rectal cancer.Chemotherapy can significantly reduce the patient’s recent social function after surgery.TCM treatment has a certain effect on improving the patient’s short-term postoperative physical condition.
