Objective: To observe the modulating effect of "Xingnao Kaiqiao" acupuncture on the permeability of the blood-brain barrier after cerebral ischemic injury,and to investigate its mechanism of action,to provide new ideas for the treatment of cerebrovascular diseases by acupuncture,and to strengthen and enhance the theory of treatment in Chinese medicine.Methods: Eighty-eight SPF male SD rats were randomly divided into blank group(N=8),sham-operated group(N=8),model group(N=24),Zhongfeng Gao group(N=24)and acupuncture+Zhongfeng Gao group(N=24)according to body weight,except for the blank and sham-operated groups,the other three groups were further divided into three subgroups of4 h,3d and 15d,8 rats in each subgroup.The blank group was left untreated,the sham-operated group was given saline gavage 2 times/day,the Zhongfeng Gao group was given Zhongfeng Gao suspension gavage 2 times/day,and the acupuncture + Zhongfeng Gao group was given the "Xingnao Kaiqiao" acupuncture method on top of Zhongfeng Gao gavage 1time/day.After the intervention,the neurological deficits of the rats in each group were measured by Zea-longa scale;the blood-brain barrier permeability of the rats in each group was evaluated by Evans blue assay;the relative expression of ICAM-1 and MMPs-9 in the ischemic brain tissue was measured by real-time quantitative polymerase chain reaction.Results:1.Neurological function deficit score of ratsIschemia-reperfusion 4h: Compared with the blank group and sham operation group,neurological deficit scores of model group,apoplexy plaster group and acupuncture and apoplexy plaster group all increased,with statistical significance(P<0.05).Ischemia-reperfusion 3d: Compared with the model group,the neurological deficit score of the apoplexy plaster group was almost unchanged,and the neurological deficit score of the acupuncture and apoplexy plaster group was decreased,but the difference was not statistically significant(P>0.05).Ischemia-reperfusion 15 days: Compared with the model group,scores of neurological function deficit were decreased in the apoplexy plaster group and the acupuncture and apoplexy plaster group,with statistical significance(P<0.05).Among them,the decrease was more significant in the acupuncture and apoplexy paste group than in the apoplexy paste group,but the difference was not statistically significant(P>0.05).Compared with 4h and 3d,the neurological function deficit score of acupuncture and apoplexy paste group on 15d was significantly decreased,and the difference was statistically significant(P<0.05).2.Determination of EB content in ratsIschemia-reperfusion 4h: Compared with the blank group and sham group,the content of EB in the model group,apoplexy plaster group and acupuncture and apoplexy plaster group all increased,with statistical significance(P<0.05).Ischemia-reperfusion 3d: Compared with the model group,the content of EB in the apoplexy plaster group and the acupuncture and apoplexy plaster group decreased,the difference was statistically significant(P<0.05),but there was no statistically significant difference between the apoplexy plaster group and the acupuncture and apoplexy plaster group(P>0.05).Ischemia-reperfusion 15 days: Compared with the model group,the content of EB in the apoplexy plaster group and the acupuncture and apoplexy plaster group decreased,with statistical significance(P<0.05).Among them,the decrease of acupuncture and apoplexy paste group was more significant than that of the apoplexy paste group,the difference was statistically significant(P<0.05).Compared with 4h,the content of EB in acupuncture and apoplexy paste group increased at 3d and decreased at 15d.3.Determination of ICAM-1mRNA in rat brain tissueIschemia-reperfusion 4h: Compared with the blank group and sham operation group,the ICAM-1mRNA content in the model group,apoplexy plaster group and acupuncture and apoplexy plaster group increased,with statistical significance(P<0.05).Ischemia-reperfusion 3d: Compared with the model group,the mRNA content of ICAM-1 in both the apoplexy plaster group and the acupuncture and apoplexy plaster group decreased,but the difference was not statistically significant(P>0.05);there was no statistical difference between the apoplexy plaster group and the acupuncture and apoplexy plaster group(P>0.05).Ischemiareperfusion 15 days: Compared with the model group,the ICAM-1mRNA content in the apoplexy plaster group and the acupuncture and apoplexy plaster group decreased,with statistical significance(P<0.05).Among them,the decrease of ICAM-1mRNA in the acupuncture and apoplexy plaster group was more significant than that in the apoplexy plaster group,and the difference was statistically significant(P<0.05).The mRNA content of ICAM-1 in acupuncture and apoplexy paste group decreased at 4h,3d and 15d successively.4.The content of MMPS-9 in rat brain tissue was determinedIschemia-reperfusion 4h: Compared with blank group and sham operation group,MMPS-9 content in model group,apoplexy plaster group and acupuncture and apoplexy plaster group increased,with statistical significance(P<0.05).Ischemia-reperfusion 3d:Compared with the model group,the content of MMPS-9 in the apoplexy plaster group and the acupuncture and apoplexy plaster group decreased,with statistical significance(P<0.05),while there was no statistical difference between the apoplexy plaster group and the acupuncture and apoplexy plaster group(P>0.05).Ischemia-reperfusion 15 days: Compared with the model group,the MMPS-9 content in the apoplexy plaster group and the acupuncture and apoplexy plaster group decreased,with statistical significance(P<0.05).Among them,the decrease of acupuncture and apoplexy paste group was more significant than that of the apoplexy paste group,and the difference was statistically significant(P<0.05).Compared with 4h,the content of MMPS-9 in the acupuncture and apoplexy paste group increased after3 days and decreased after 15 days.Conclusion:1." Xingnao Kaiqiao" acupuncture has a bidirectional regulation effect on the blood-brain barrier permeability of cerebral ischemia injury.After 3 days of cerebral ischemia and reperfusion,"Xingnao Kaiqiao" acupuncture can reduce the permeability of BBB,and after15 days,"Xingnao Kaiqiao" can increase the permeability of BBB.2.This bidirectional regulation may be achieved by acupuncture regulating the content of ICAM-1 and MMPS-9 in the brain tissue of rats with ischemia/reperfusion.
