Objective:To explore the related factors that affect the relapse of NMOSD patients.Methods:The clinical data of 69 patients diagnosed with Neuromyelitis optica spectrum disorder at the first incidence were collected.The patients were divided into the initial group(25 patients)and the relapse group(44 patients)according to whether the disease recurred.To analyze the data,first use the(χ~2 test to discuss the relationship between various factors and disease recurrence.and binary Logistic regression analysis was used for statistically significant factors to explore independent influencing factors related to NMOSD recurrence.Then according to the number of relapses it will be divided into 3 groups:0 relapse,1-3 relapses,and>3 relapses.the ordinal logistic regression analysis was used to explore the correlation with the relapse frequency.Results:The age of onset and gender,AQP4 antibody,Extended Disability Status Scale(EDSS)≥5,autoimmune antibody,chole-sterol(CHO),low density lipoprotein cholesterol(LDL-C).There are statistically significant differences(P<0.05);there is no intracranial lesion,spinal cord involvement≥6 segments,uric acid level,cerebro-spinal fluid protein,cystatin C,C reactive protein between the two groups There was no statistical difference(P>0.05);the proportion of women in the initial group and the relapse group were 50.00%and 77.78%,respectively;there was a statistical difference between the two groups((χ~2=5.561,P<0.05);The positive rates of AQP4antibody in the relapse group were 16.00%and 75.00%,respectively;there were statistical differences between the two groups((χ~2=22.31,P<0.01);the proportion of late-onset(>50 years old)in the initial group and the relapse group was Were 20.00%and 56.82%,the difference between the two groups was statistically significant((χ~2=8.79,P<0.01);the positive rates of autoimmune antibodies in the initial group and relapse group were 56.00%and 93.18%,respectively.Academic difference((χ~2=13.63,P<0.01);the proportion of people with high CHO in the initial group and relapse group were 20.00%and 54.55%,respectively,and there was a statistical difference between the two groups((χ~2=7.81,P<0.01);the proportion of patients with high LDL-C in the initial group and relapse group were 12.00%and 52.27%,respectively,and there was a statistical difference between the two groups((χ~2=11.01,P<0.01);the initial group and relapse The proportion of people with EDSS≥5 in the group was 28.00%and 54.55%respectively.There was a statistical difference between the two groups((χ~2=4.54,P<0.05).Binary logistic regression analysis showed that AQP4 antibody was positive(P=0.004,OR=8.658,95%CI=1.994~37.588);positive autoimmune antibodies(P=0.026,OR=9.245,95%CI=1.303~65.596)were associated with recurrence within one year,and further ordinal logistic regression analysis showed that AQP4 antibody positive and autoimmune antibody positive were the factors affecting the frequency of recurrence.Conclusion:AQP4 antibody positive and autoimmune antibody positive are independent risk factors for NMOSD relapse within 1 year,and may be positively correlated with the frequency of relapse,which can be used as indicators to predict clustered attack occurrence of NMOSD.
