| objectiveto investigate the clinical effects of high-intensity non-invasive positive pressure ventilation(HI-NPPV)in the treatment of AECOPD patients complicated with typeⅡrespiratory failure.Methods72 cases of AECOPD patients with typeⅡrespiratory failure treated in our hospital from October 2018 to January 2020 were selected,and randomly assigned to a group receiving high-intensity NPPV with inspiratory positive airway pressure(IPAP)of(24.7±1.8)cm H20,or to a group receiving conventional pressure NPPV(CP-NPPV)with IPAP of(16.1±1.2)cm H20.The value of p H、PaCO2、PaO2 in arterial blood at different-point was analyzed before and after treatments.The adverse reaction、tracheal intubation rate、mortality rate、hospital stays were analyzed.The 6-minutes walking distance(6MWD)in the day of leaving hospital and readmission rate during28 days out of the hospital were compared between the two groups as well.ResultsAfter treatments,in the group of HI-NPPV,the value of PaO2 increased much more significantly while the value of PaCO2 were markedly decreased(P<0.05).The value of p H shows no difference between two groups.When compared with the group of CP-NPPV,the tracheal intubation rate、mortality rate and hospital stays were significantly lowered in group HI-NPPV(P<0.05).The readmission rate in 28 days after discharge and the distance in 6MWD were no significantly difference between two groups(P﹥0.05).The adverse reaction was not increased or much worse when using the HI-NPPV.ConclusionThe application of HI-NPPV in the treatment of COPD patients complicated with typeⅡrespiratory failure could improve the blood gas analysis index much more effectively.It decreased the tracheal intubation rate、mortality rate and hospital stays as well.In the meantime,it did not increase the adverse reactions. |
