Objectives We aimed to assess the current status of hip joint function and self-care agency in elderly patients with hip fractures 3 months after operation,analyze the correlation between hip joint function and self-care agency in elderly patients with hip fractures 3 months after operation,explore the factors affecting the recovery of joint function in elderly patients with hip fractures 3 months after operation and further provide a theoretical reference for the intervention research to improve the joint function of elderly patients with hip fracture.Methods This study is a cross-sectional study,we used the convenient sampling method.A questionnaire survey was conducted on elderly patients who were rechecked/followed up for 3 months after operation for hip fractures in the Orthopedic Ward of Yan’an University Affiliated Hospital and Yan’an People’s Hospital from January 2020 to January 2021.In this study,the general information questionnaire,Harris Hip Score(HHS)and Exercise of Self-Care Agency scale(ESCA)were used to analyze the general information of patients with hip fractures and postoperative hips.We collected data on joint function and self-care agency.Epi Data 3.1 is used for data entry.After verification,SPSS25.0 software is used for data analysis,and Graph Pad Prism 8.0.2 is applied for drawing software.The Kolmogorov-Smirnov test and Shapiro-Wilk test were used to test the normality of the data of the research subjects and the results were skewed and expressed in percentiles.The general information of the research object is described by frequency and percentage.The non-parametric Mann-Whitney U test was used for comparison between two groups,the Kruskal-Wallis H test was used for comparison between multiple groups and Bonferroni was further used for pairwise comparison.Spearman rank correlation analysis,multiple linear stepwise regression analysis.While the difference was statistically significant(P<0.05).Results Among the 231 subjects of this survey,8 cases were cancelled due to other reasons,and 13 cases were cancelled due to changes in their contact numbers,hence unable to be contacted.So 210 questionnaires were actually returned.The recovery rate: 90.91%.1.The median score of hip joint function in 210 elderly hip fractures at 3 months after operation was 64 points,2 cases were excellent,accounting for 1.0%.48 cases were moderate,accounting for 22.8%,and 160 cases were poor,accounting for 76.2%.The median score of self-care agency of the research subjects at 3 months after operation was 89 points.The median scores of the four dimensions of self-care ability were 15 points for self-concept,12 points for self-care responsibility,25 points for self-care skills,and 37 points for health knowledge level.Patients in this period have the highest level of health knowledge,followed by self-care skills,self-concept and self-care responsibility.There were moderate level of 208 cases(99.0%),the high level was 2 cases(1.0%).2.The results of Spearman rank correlation analysis showed that there was a significant positive correlation between the hip joint score and the self-care agency score and various dimensions of patients 3 months after operation(P < 0.01),self-concept(rs=0.779,P < 0.001),self-care responsibility(rs=0.656,P < 0.001),self-care skills(rs=0.726,P < 0.001),health knowledge level(rs=0.738,P < 0.001),self-care agency(rs=0.788,P<0.001).3.The results of the non-parametric Mann-Whitney U test and Kruskal-Wallis H test showed that the joint function of elderly hip fracture patients had significant on living area,age,marital status,education level,caregiver,surgical location,implant/prosthesis material type the difference was statistically significant(P<0.05).Multiple linear stepwise regression analysis showed that the main factors affecting the joint function of elderly patients with hip fracture were: age,caregiver,marital status,education level,implant/prosthesis source,and self-care agency.These 6 variables accounted for 89% of the variation in hip joint function of the study subjects.Conclusions 1.In this study,the results of this survey showed that the joint function of elderly patients with hip fractures is not ideal for 3 months after operation and is expected to be further improved.Majority of the elderly patients with hip fractures have a moderate level of self-care agency after operation.2.There was a significant positive correlation between the total score of hip joint function and the total score of self-care agency in elderly patients with hip fracture at 3 months after operation.3.Age and caregivers are negatively correlated with joint function in elderly patients with hip fractures 3 months after operation.Divorce in marital status,imports of implant/prosthesis materials,education level and self-care agency are positively correlated with joint function of elderly hip fracture patients 3 months after operation.Based on the main influencing factors in the results of this survey,the responsible nurses can provide targeted,personalized and professional rehabilitation nursing measures to improve the patient’s self-care level to provide appropriate nursing programs for the joint function of elderly hip fracture patients and to improve the prognostic quality of life and selfconfidence of patients.
