| ObjectiveIn this study,we compared the clinical efficacy of Wendan Decoction Combined with laser photocoagulation and laser photocoagulation alone in the treatment of DR,the difference of TCM clinical syndrome score and the change of NF-k B level;explored whether Wendan Decoction had synergistic effect on the improvement of clinical manifestations of DR;whether the mechanism of synergistic effect was related to inflammatory reaction;enriched the clinical treatment of DR,and provided theoretical basis for it.MethodCollected 60(107 eyes)cases of DR patients with phlegm-stasis blockade in the Department of Ophthalmology,People’s Hospital of Fujian University of Chinese Medicine.Randomly divided into treatment group(30 cases,56 eyes),control group(30 cases,51 eyes).The treatment group was treated with Wendan Decoction combined with laser photo-coagulation treatment plan,and the control group was treated with laser photo-coagulation treatment plan alone.Observe the differences of groups from aspects of BCVA,Retinal hemorrhage,exudation area and microvascular tumor density in fundus photography,CMT,In FFA examination,the leakage point of neovascularization,the density of microvessel tumor,the degree of fluorescence accumulation in the late macular area,and the area of non perfusion area were detected,and non-perfusion area were compared with overall clinical efficacy,Serum NF-κB expression level,differences in TCM clinical syndrome scores of the two groups before and after treatment.Result1.The results of statistical analysis showed that there was no statistical significance in the comparison between the two groups in terms of age distribution,gender difference,clinical staging,BCVA level,CMT,serum NF-k B expression level,and TCM clinical syndrome score before treatment(P >0.05),the data are comparable.2.Clinical efficacy analysis: After treatment,BCVA,Retinal hemorrhage,exudation area,microvascular tumor density absorption,FFA were all improved compared with before treatment,and the difference was statistically significant(P<0.05);the efficacy of the treatment group was better than that of the control group,The difference was statistically significant(P<0.05);CMT decreased compared with the previous,the difference was statistically significant(P<0.05),the treatment group was more obvious,the difference was statistically significant(P<0.05);treatment group,control group The total curative effect was91.1% and 80.4%,respectively,the treatment group was better,and the difference was statistically significant(P<0.05).3.Comparison of serum NF-k B level: NF-k B expression decreased after treatment,and the difference was statistically significant(P<0.05).The decrease in the treatment group was even more significant,and the difference was statistically significant(P<0.05).4.Comparison of TCM clinical syndrome scores: After treatment,the TCM clinical syndrome scores of the two groups were lower than before treatment,which was statistically significant(P<0.05),and the treatment group was more significant and statistically significant(P<0.05).In the comparison of the curative effect of the integral ratio,the treatment group and the control group were 93.3% and 10.0%,respectively.The clinical improvement of the treatment group was better,which was statistically significant(P<0.05).Conclusion1.After treatment,the clinical efficacy and TCM clinical syndrome scores of the two groups were lower than before treatment.Compared with laser photo-coagulation treatment alone.Wendan Decoction combined with laser photo-coagulation treatment has better curative effect on phlegm-stasis-blocked DR,Wendan Decoction has a synergistic effect on the clinical treatment of DR.The safety observation showed no obvious abnormality,and the treatment was safe and reliable.The two groups of treatment had a positive effect in reducing the expression of serum NF-k B;the effect of Wendan Decoction Combined with laser photocoagulation on reducing the level of NF-k B was more significant.It is speculated that the mechanism of Wendan Decoction’s synergistic effect may be to reduce the level of serum NF-k B by inhibiting the inflammatory reaction in vivo. |
