Objective To analyze the distribution and attribution of 24 HPV(Human Papillomavirus)subtypes in SIL(cervical squamous intraepithelial lesion)and CC(cervical cancer),so as to provide reference for screening and prevention of CC.Methods A retrospective analysis was carried out in 541 patients with cervical lesions who were treated at the Minda Hospital of Hubei Minzu University from January 2018 to December 2020,they were divided into LSIL(low-grade cervical squamous intraepithelial lesions),HSIL(high-grade squamous intraepithelial lesion)and CC groups according to their histopathological results.The HPV subtypes and clinical information were obtained for analysis.Results 1.The total positive rate ofHPV in all patients was 93.90%,among which the high-risk type was 84.47%,the high-risk and low-risk mixed type was 6.28%,and the low-risk type was 3.14%(P<0.05).2.The positive rates of HPV in LSIL,HSIL and CC groups were 91.00%,94.27% and 95.07%(P>0.05);the constituent ratios of single infection were 69.31%,75.23% and 87.05%,and the constituent ratios of multiple infections were 30.69%,24.77%,and 12.95%(P<0.05).3.The subtypes of HPV infectionin in three groups are mainly high-risk infections.Among the LSIL individuals,the most prevalent HPV subtypes was HPV 16,and the 4 common subtypes in decreasing order were HPV 18,52,58,and 66.Of the HSIL patients,the most frequently HPV subtype was HPV 16 followed by 58,52,18 and 33.Among the CC patients,for HPV genotypes,HPV 16,58,18,52 and 56 were five most common subtypes.The number of HPV-negative cases in each group were 9.00%,5.73%,and 4.93%.4.The distribution of common single infection of HPV subtypes in the LSIL,HSIL and CC groups: HPV16: 19.11%,36.84% and 69.10%(P<0.05);HPV18: 14.71%,5.85% and 7.87%(P>0.05);HPV58:14.71%,21.64% and 7.87%(P<0.05);HPV52:13.24%,11.11% and 3.93%(P<0.05).5.The infection rate of HPV in different age groups showed a U-shaped distribution,with two peak age ranges of distribution,it contained age < 25 years old and ≥ 55 years old.6.There was no statistically significant difference between the types of cervical lesions between Han and ethnic minorities in Enshi area(P>0.05).7.There were statistically significant difference in the HPV16/18 positive,pregnancy times,parity times,abortion times,menopausal between CC group and LSIL/HSIL group(P<0.05).8.The cumulative attribution rates of HPV 16,18,58,52,33,31 and45 in LSIL,HSIL and CC were 48.4%,72.3% and 88.3%.The top four subtypes in the attribution rate were HPV 16,58,18 and 52.Conclusion 1.The HPV infection rate of patients with cervical lesions in Enshi area is 93.90%.The infection types are mainly single and high-risk types.The common subtypes are HPV 16,58,18 and 52;HPV negative also accounts for a certain proportion.2.High-risk HPV 16/18 positive,pregnancy times ≥2 times,parity ≥2 times,abortion times ≥2 times,and menopause are high risk factors of CC.3.HPV16,18,58,52,33,31 and 45 have a high prevalence,cumulative attribution rates and risk values in patienrs with cervical lesions,it is suggested that the HPV nine-valent vaccine should be preferred for the women in Enshi area.
