Research Purpose:The purpose of this study is to construct a transitional of family integrated care programme for patients having breast cancer surgery based on Omaha System,and to explore the effects of this programme on the shoulder joint mobility,the occurrence of lymphedema of the affected limb,the self-care ability and the quality of life of breast cancer patients,and the family members The impact of caring capacity and quality of life.It is expected to provide practical reference and theoretical basis for future family integrated and transitional care research of breast cancer patients.Research Methods:The first part of the study is based on the Omaha system and family integrated care.Through literature review,book reading,semi-structured interviews and group discussions,The transitional of family integrated care programme for patients having breast cancer surgery based on Omaha System is initially drawn up.And use the Delphi method to determine the final draft of the plan.In the second part of the study,a randomized controlled experiment was adopted.Patients and their families who were admitted to the breast surgery department of a tertiary hospital in Huzhou City,Zhejiang Province from July to December 2020 were used as the research objects.The group randomized method was used to divide them into the control group and the experimental group.The control group was given a routine continuation care plan,and the test group was given a transitional of family integrated care programme for patients having breast cancer surgery based on Omaha System after breast cancer surgery.on the 7th day,1 month and 3months after the operation.Collect the patient’s shoulder joint mobility,the occurrence of lymphedema of the affected limb,self-care ability and quality of life and the care ability and quality of life of the family members data.And through SPSS23.0 software using chi-square test,t test,repeated measures analysis of variance and Mann-Whitney U test and other methods for statistical analysis of the data.Research Results:In the two rounds of expert consultation,the experts’ authority coefficients(Cr)were 0.886 and 0.906,and the enthusiasm of experts was 88.2% and 100%,respectively.The Kendall’s W values of the experts were 0.233-0.358 and 0.326-0.383(P<0.01).The CV values of experts are0.09-0.42 and 0.08-0.27,respectively.The final form of family participatory breast cancer postoperative continuation nursing program based on the Omaha system includes 3 categories,13 nursing problems,and 45 corresponding nursing interventions,including psychosocial fields,physiological fields,and health-related knowledge.The study subjects were divided into groups using a block random method.Among them,36 pairs of the control group and 35 pairs of the experimental group completed the study.The intervention period of the study was from the discharge of the patient to 3 months after the operation.(1)The comparison of general data between the two groups of subjects was not statistically significant(P>0.05).(2)Seven days after surgery,there was no statistical difference between the two groups of patients in the range of motion of the shoulder joint,the occurrence of lymphedema,the ability of self-care,and the quality of life(P>0.05).(3)At 1 month and 3 months after operation,the shoulder joint range of motion,lymphedema occurrence,self-care ability and quality of life of the test group were better than those of the control group,the difference was statistically significant(P<0.05).(4)Seven days after the operation,the care ability and quality of life of the family members of the two groups were compared,and the results were not statistically different(P>0.05).(5)At 1 month and 3 months after operation,the care ability and quality of life of the family members of the test group were better than those of the control group,the difference was statistically significant(P<0.05).Research Conclusion:(1)The family participatory continuation care program for breast cancer surgery based on the Omaha System the experts are highly motivated,authoritative,coordinated,and concentrated in expert opinions during the two rounds of expert consultation.Therefore,the intervention program is authoritative and scientific.(2)The family participatory continuation care program for breast cancer surgery based on the Omaha system can improve the mobility of the affected limbs and shoulder joints and lymphedema,and can improve the self-care ability and quality of life of breast cancer patients.(3)The family participatory postoperative continuation care program for breast cancer based on the Omaha system can improve the care ability and quality of life of the family members.
