Construction Of Prevention Strategy Of Aspiration During Nasal Feeding In Elderly Hospitalized Patients

Research Purpose:Based on evidence-based and Delphi method to construct a preventive strategy of aspiration in elderly inpatients during nasal feeding.To reduce the nasal feeding of old-age patients and other related complications in my country.To improve the treatment of clinical nasal feeding and quality of nursing services Provide a reference basis for the implementation of nasalification standardization management for hospitalized elderly patients.Research Methods:1.Evidence synthesis: The system’s search for clinical practice guidelines,optimal evidence,system evaluation,and primitive research of nasal fertile suction prevention related to hospitalized elderly patients.Screening literature;Used the UK AGREE II Guide Evaluation Tool and JBI Document Quality Evaluation Standard(2016)to quality evaluation;Used the JBI(2014)Evidence preprices and evidence recommendation level system to evidence,summarize;Developed strategy first draft.2.Delphi method: Select 20 clinical experts to carryed out 2 rounds of experts,and statistically analyze the enforcement results to established the final strategy.Research Results:1.The strategy finally incorporated in a total of 31 guidelines and articles,included 25 grade A,and 6 grade B.Summary Evidence,the first draft of the inclusion prevention strategy during the nasal suction prevention strategy of hospitalized elderly patients included 5 first-level items,19 second-level items,and 89 third-level items were summarized.2.The effective recovery volume of the two-round expert consultation was 100% and 95%,respectively;the authority coefficient was 0.883 and 0.894;the importance of the importance variation coefficient was 0.00～0.13,the coefficient of operability variation was 0.00～0.20;the coordination coefficient of expert informs was 0.336～0.571(P <0.01);summary expert opinion and suggestion,discussed by the research team,ultimately formation of Prevention strategy of aspiration during nasal feeding in elderly hospitalized patients which included 5 first-level items,20 second-level items,and 90 third-level items.Research Conclusion:This study constructs a prevention strategy of aspiration during nasal feeding in elderly hospitalized patients,based on Evidence-based and Delphi method.The strategy includes five dimensions: identification of aspiration risk factors,assessment of aspiration,intervention measures,education and training,and continuous quality improvement.The strategy construction process is scientific and rigorous,and the results of Delphi method are coordinated and reliable.Providing best preventive measures for clinical nursing staff to improve the effectiveness and safety of nasal feeding in elderly patients.
