| Objective: To study the expression of HIF-1α and PKM2 in Hepatolithiasis-associated intrahepatic cholangiocarcinoma(HICC)and their relationship with clinicopathological features,and comparing and analyzing the expression of HIF-1α and PKM2 in Non-hepatolithiasis associated intrahepatic cholangiocarcinoma(NHICC).So as to understand the mechanism of the occurrence and development of HICC and NHICC,and provide a new theoretical basis for clinical diagnosis and treatment.Moreover,the potential diagnostic and therapeutic value of PKM2 and HIF-1α in ICC was preliminarily understood.Method: A total of 54 cases of pathological tissue specimens of patients that underwent surgical resection due to intrahepatic cholangiocarcinoma in our hospital from January 2012 to December 2020 were collected,including 26 cases in HICC group and 28 cases in NHICC group.A total of 15 cases of pathological tissue specimens for patients that underwent hepatectomy due to intrahepatic bile duct stones with cholangitis,and 15 cases of normal bile duct tissue specimens were collected as control group.Immunohistochemical method(Envision method)was used to detect the expression of HIF-1 and PKM2 in 54 cases of ICC tissues,15 cases of intrahepatic bile duct stone hepatobiliary tissue and 15 cases of normal hepatobiliary tissue,and then the expression level and its relationship with clinicopathological features were analyzed.Moreover,the influence of inflammatory reaction on the expression of HIF-1α and PKM2 was analyzed.SPSS24.0 statistical software was used for analysis.Results:(1)The positive expression rates of HIF-1 and PKM2 in ICC were 77.78%(42/54)and 70.37%(38/54),respectively.The positive expression rates of HIF-1 and PKM2 in normal bile duct tissues were26.67%(4/15)and 20.00%(3/15),respectively.The difference was statistically significant(P<0.05).(2)The positive expression rates of HIF-1 and PKM2 in HICC were 80.77%(21/26)and 73.08%(19/26),respectively,and the expression rates of HIF-1 and PKM2 in NHICC were 75.00%(21/28)and 67.86%(19/28),respectively,which were significantly different from those of HIF-1 and PKM2 expression in normal bile duct tissues(P<0.05).(3)The expression of HIF-1 and PKM2 in ICC was positively correlated with lymph node metastasis and TMN stage(P<0.05).(4)The expression of HIF-1α in HICC was positively correlated with lymph node metastasis,serum CEA and CA199(P<0.05).The expression of PKM2 in HICC was positively correlated with lymph node metastasis and TMN stage(P<0.05).(5)The expression of HIF-1αin NHICC was positively correlated with the serum neutrophil ratio(P<0.05).The expression of PKM2 in NHICC was positively correlated with lymph node metastasis and TMN stage.(6)There was a positive correlation between the expression of HIF-1 and PKM2 in ICC(P<0.05).However,there was no correlation between the expression of HIF-1 and PKM2 in HICC group and NHICC group(P>0.05).Conclusion:(1)There is no difference in the expression of HIF-1αand PKM2 in HICC and NHICC tissues,but the expression of HIF-1αand PKM2 in ICC,HICC and NHICC tissues is significantly higher than that in normal bile duct tissues,which indicates that there is a certain diagnostic and therapeutic value.(2)The expression of HIF-1α and PKM2 in ICC,HICC and NHICC tissues is positively correlated with tumor invasion and metastasis,indicating that they may be predictive Important factors for tumor prognosis.(3)The expression of HIF-1α and PKM2 in ICC is positively correlated,indicating that the two can be combined Used in diagnosis and treatment of ICC. |
