| Objective(s):Chronic photodermatitis is a group of chronic spectrum skin diseases characterized by photosensitivity,caused by long-term exposure to ultraviolet light;and chronic dermatoses are a group of diseases that seriously affect human physical and mental health,most of which have unknown or complex etiology,inaccurate efficacy,lack of specific treatment options,and a few serious cases can endanger the safety of patients themselves and society.Most chronic skin diseases are characterized by skin lesions,often accompanied by pain,itching and different degrees of skin mucosal damage,and patients often suffer from depression,anxiety and other psychological problems,and even distrust of medical workers and refuse to accept treatment.This study will analyze the age,gender,lesion characteristics,and psychological characteristics of the patients,as well as some of the patients treated with photosclerosis.Ultimately:(1)to clarify the clinical characteristics,possible causative factors and psychological and sociological characteristics of patients with chronic actinic dermatitis in Yunnan;(2)to clarify the clinical characteristics and possible causative factors of patients with chronic actinic dermatitis with different skin manifestations.(3)To observe the efficacy of photosclerosis treatment.Methods:1.A total of 150 patients with CAD(104 cases after removing invalid cases,and a small number of blank controls)were collected from the Department of Dermatology of the First Affiliated Hospital of Kunming Medical University and the Department of Dermatology of some local and state hospitals from May 2020 to April 2021,and signed informed consent forms;investigators assisted patients to fill out questionnaires together(for details of the questionnaires,please refer to the annex"Questionnaire on chronic actinic dermatitis 2.The sociological data,photosensitivity tests,treatment parameters and clinical responses of patients with chronic actinic dermatitis treated with photosclerosis in the Department of Dermatology,The First Affiliated Hospital of Kunming Medical University from December 2020 to April 2021 were analyzed.results,treatment parameters and clinical responses were analyzed.SPSS23 software was used to compare the clinical data of patients who underwent 0,5 and 10 sessions of photosclerosis treatment.Results:In this study,there were 80 male patients and 24 female patients,the number of male patients was much higher than the number of female patients,and the ratio of male to female was 10:3.The proportion of patients whose occupation was farmer was the highest,41.3%,while the proportion of full-time students and employees working mainly indoors was lower,9.6%.The majority of patients had the onset in March and June,which is consistent with the high incidence of CAD in spring and summer.Fifty-two patients had a clear history of exposure to photosensitizing substances,and 25 patients were unsure whether they had been exposed to photosensitizing substances.Thirty-one consumed photosensitive foods such as ashwagandha,purple clover,figs,celery,fungus,and mud snails;exposure to internal medications,detergents,halogenated N-salicylated aniline(TCSA)in soap,and topical medications was also high;and more than 60%of patients had smoked or were smoking.In contrast,55.8%of patients(58)never drank alcohol,close to 60%washed their faces without using cleansers,more than 70%had no habit of using moisturizing skin care products,and close to 60%used or used sun protection measures occasionally when going out,but mostly physical sun protection(duck-tongue hats,straw hats,etc.)and used them infrequently,and rarely used sunscreens or shades.The DLQI score was 1 for the lowest total score and 29 for the highest score.The mean score of 104 patients was 12.34.Of the 104 patients’ satisfaction scores with their current skin condition,the mean score was 56.88,with a maximum value of 100 and a minimum value of 0.After analyzing the past medical history of different types of skin damage using the chi-square test,the p-value obtained from the test was 0.041,which was less than 0.05,indicating that there were significant differences in the past medical history of different skin damage types.Specifically,the proportion of patients with a past medical history was significantly higher in the erythema type than in the eczema and itchy rash types.In terms of photosclerosis treatment,with a starting dose of 0.10 j/cm2 and an endpoint dose of 0.13 j/cm2 and slowly increasing dose,the number of photosclerosis treatments for patients was positively correlated with the degree of skin damage recovery,with no significant correlation with patients’ age and photosensitivity,and with a low incidence of intolerance,and photosclerosis treatment can be recommended for patients of all ages.Conclusion:1.The incidence ratio of men to women with chronic photodermatitis was 10:3,similar to previous studies.2.50%of patients had a clear history of photosensitive substance exposure,mostly from the consumption of photosensitive foods.3.more than 60%of patients had smoked or were smoking and had a high risk of behavioral sunburn.4.the mean score of the DLQI score was 12.34,indicating that the disease had a heavy impact on the patients’ quality of life.5.The proportion of erythematous patients with a past medical history was significantly higher than the proportion of eczema and itchy rash patients.6.The post-onset sun exposure time was significantly higher in the erythematous and itchy rash types than in the eczema type.7.Photohardeningwas effective in improving the condition of the patients,and the degree of improvement was higher in patients treated with photohardening than in those who did not undergoing. |
