Study On The Distribution Law Of TCM Constitution With Ischemic Stroke Patients And Its Correlation With Color Doppler Ultrasound Parameters Of The Radial Artery In Cunkou Region

Objective: This paper aims at the core pathogenesis that ischemic stroke is brain dysfunction caused by vascular changes,starting from the TCM constitution,applying color Doppler ultrasound,to explore the TCM constitution distribution of patients with ischemic stroke and the hemodynamic changes of the radial artery at cunkou between different constitutions,improve the study of TCM physique of patients with ischemic stroke,and realize the objective and quantitative expression of TCM pulse diagnosis technology.Methods: The overall study adopted a cross-sectional survey,using the “ischemic stroke case report form”(CRF form)and the "Traditional Chinese Medicine Classification and Judgment Form"(China Association for TCM ZZYXH/T157-2009)to collect general patient data and determine the constitution of stroke patients.Then,the Doppler Philips IE EIite color ultrasound system was used to directly observe the changes of the radial artery vascular structure and blood flow in patients with ischemic stroke under different TCM constitutions,obtain relevant data such as the distance from the skin to radial artery at the Cun-kou area,the inner diameter of the blood vessel,PS,ED,Tamax,PI,RI and other related data.Result:1.The common TCM constitutions of 90 patients with ischemic stroke were: phlegm-dampness,qi deficiency,qi stagnation,and damp-heat.2.Using color Doppler ultrasound technology to monitor the hemodynamic changes of the cunkou,the constitution of deficiency of Qi,Qi stagnation,phlegm-dampness,damp-heat constitution of patients with ischemic stroke is on PS,ED,PI,RI and the inner diameter of blood vessels There is a significant difference,and there is no statistical difference in the distance from the skin to the radial artery and Tamax.3.In terms of blood flow speed,patients with ischemic stroke have the fastest damp-heat constitution PS and ED,Qi deficiency constitution PS the slowest,and ED phlegm dampness the slowest.Damp-heat quality PS is significantly different from phlegm-damp quality,Qi-deficiency quality,and Qi-stagnation quality.Phlegm-damp quality ED is significantly statistically significant compared with wet-heat quality,Qi-deficiency quality and Qi-stagnation type;In terms of pulsation index,the PI of phlegm-dampness quality and damp-heat quality was the highest,and the PI of Qi deficiency quality was the lowest.The PI of Qi deficiency and phlegm-dampness,damp-heat and qi-stagnation are statistically significant,and the PI of Qi-stagnation,phlegm-dampness,and damp-heat quality are also statistically significant.There is no statistical difference between the damp-heat and phlegm-dampness quality;In terms of resistance index,phlegm-damp quality RI is the highest,followed by damp-heat quality and Qi stagnation quality,and Qi-deficiency quality RI is the lowest.Qi-deficiency quality and damp-heat quality,phlegm-damp quality,and Qi-stagnation RI are statistically significant,and Qi-stagnation quality and damp-heat quality RI are statistically significant;In terms of the inner diameter of blood vessels,the qideficiency type has the thickest vessel,and the remaining qistagnation,phlegm-dampness,and damp-heat quality have no significant difference in the inner diameter of the blood vessel.Conclusion:1.The common TCM constitution of patients with ischemic stroke is phlegm-dampness,followed by qi deficiency,qi stagnation,and damp-heat.2 There are differences in the parameters of the radial artery between different constitutions of ischemic stroke patients,which may provide a reference index for the differential diagnosis of ischemic stroke patients with different constitutions,but further large sample studies are still needed.
