Clinical Study On The Prevention And Treatment Of Depressive State After Acute Myocardial Infarction By Yiqi Huoxue Decoction

1.Analysis of influencing factors of depressive state complicated after acute myocardial infarctionObjective:By collecting clinical cases,explored the factors that may affect the incidence of depressive state in patients with acute myocardial infarction,so as to provide reference for the early clinical identification of high-risk population with myocardial infarction complicated with depressive state.Methods:Patients with acute myocardial infarction from May 2020 to March 2021 were collected.general data and clinical data of the included patients were collected,including age,sex,smoking history,BMI,education level,IL-6,IL-8,peak troponin,myocardial infarction type,and LVEF.The Hamilton Depression Scale(HAMD)was used to evaluate the depression scores of the included patients.The score of the included patients was greater than or equal to 8 points,which was considered to be depressive state.SPSS 20.0 software was used for single-factor and multiple-factor analysis.Results:① 100 patients with acute myocardial infarction were enrolled,among which 32 patients were complicated with depressive state and 68 patients were not complicated with depressive state.The incidence of depressive state in patients with acute myocardial infarction was 32.0%.② Single-factor analysis showed that age,gender,IL-6 and LVEF affecting the incidence of depressive state in patients with acute myocardial infarction(P<0.05),while smoking history,BMI,educational level,IL-8,peak troponin and myocardial infarction type were not associated with the morbidity of depressive state in myocardial infarction patients(P>0.05).③ Multiple-factor analysis showed that venerable age,female,higher IL-6 and lower LVEF were associated with acute myocardial infarction patients complicated with depressive state(P<0.05).Conclusion:Patients with acute myocardial infarction were more likely to develop depressive state in older age,women,and those with higher IL-6 and lower LVEF.Therefore,we should pay attention to the psychological problems of these people in clinical work.2.Clinical study on the prevention and treatment of depressive state after acute myocardial infarction by Yiqi Huoxue DecoctionObjective:In order to reduce the incidence of depressive state after acute myocardial infarction,early intervention was conducted on patients with acute myocardial infarction with Yiqi Huoxue Decoction to explore the effects of this recipe on the depression scale scores,the incidence of depressive state,the level of inflammatory factors,angina score and traditional Chinese medicine(TCM)syndrome score.It provides a new idea for the prevention and treatment of depression after acute myocardial infarction with integrated Chinese and western medicine.Methods:After screening according to inclusion criteria and exclusion criteria,44 patients from May 2020 to March 2021 were finally included.They were stochastically assigned to control group and experimental group of 22 each.The control group was given standard treatment,and the experimental group was given standard treatment combined with Yiqi Huoxue Decoction.After 1 week of treatment,IL-6 and IL-8 were detected in both groups and after 3 weeks of treatment,the depression scale score,the incidence of depressive state,angina score,TCM syndrome score and TCM syndrome improvement rate were evaluated.Results:① There was no prominent discrepancy of depression score in control group before and after intervention(P=0.065).There was prominent discrepancy of depression score in experimental group before and after intervention,that is,after the intervention was lower(P=0.019).There was no prominent discrepancy of depression score between two groups after intervention(P=0.128).② There was no prominent discrepancy of morbidity of depressive state between two group after intervention(P=0.262).③ There was prominent discrepancy of IL-6 in two groups before and after intervention,that is,after the intervention were lower(P=0.033,P=0.002).There was prominent discrepancy of IL-6 between two group after intervention,that is,experimental group was lower(P=0.002).④There was prominent discrepancy of IL-8 in control group before and after intervention,that is,after the intervention was lower(P=0.031).There was no prominent discrepancy of IL-8 in experimental group before and after intervention(P=0.149).There was no prominent discrepancy of IL-8 between two groups after intervention(P=0.166).⑤ There was prominent discrepancy of angina score between two groups after intervention,that is,experimental group was lower(P=0.046).⑥ There was prominent discrepancy of TCM syndrome score in two groups before and after intervention,that is,after the intervention was lower(P<0.001,P<0.001).There was prominent discrepancy of TCM syndrome score between two groups after intervention,that is,experimental group was lower(P=0.004).⑦There was prominent discrepancy of TCM syndrome improvement rate between two groups after intervention,that is,experimental group was higher(P=0.014).Conclusion:On the basis of the standard treatment of western medicine,early combined use of Yiqi Huoxue Decoction can significantly reduce IL-6,relieve angina pectoris symptoms and improve TCM syndrome in patients with acute myocardial infarction,and have a mild reduction effect on depression score and the incidence of depressive state,but it does not reach statistical significance,which needs to be further confirmed in future studies.
