Objective:To explore the expression of heat shock transcription factor 1(HSF1)in esophageal squamous cell carcinoma(ESCC)and its effect on the sensitivity of ESCC to radiotherapy and chemotherapy.Methods:(1)92 patients were divided into 4 groups:10 patients in the esophageal dysplasia group,20 patients in the ESCC I and II surgical resection group,18 patients in the ESCC Ⅲ and Ⅳ surgical resection group,there were 44 patients in ESCC Ⅲ and IV radiotherapy and chemotherapy group.Among them,the ESCC Ⅲ and IV radiotherapy and chemotherapy components included 16 cases in the HSF1 low expression group and 28 cases in the high expression group.Control group:esophagitis group 10 patients.(2)Take each group of pathological wax blocks to detect the expression of HSF1 in each group by immunohistochemistry and staining techniques and analyze the relationship between HSF1 expression and the clinical characteristics of ESCC patients.(3)Through the changes of non-tumor lesion size,tumor diameter and CEA value of HSF1 low expression group and high expression group of ESCC Ⅲ and Ⅳ patients before and after radiotherapy and chemotherapy,assess the sensitivity of patients to radiotherapy and chemotherapy.(4)Analyze the relevant factors that affect the 3-year survival of ESCC patients;study the 1-year,2-year,and 3-year overall survival rates of patients in the ESCC Ⅲ and Ⅳ radiotherapy and chemotherapy group;analyze the 3-year overall survival rate of ESCC patients.Results:(1)HSF1 was highly expressed in each ESCC group,lowly expressed in the esophagitis group and esophageal dysplasia group,and there were differences in the expression of HSF1 between each ESCC group and the esophagitis group and esophageal dysplasia group,P=0.001.there was no difference in HSF1 expression between the ESCC groups,P>0.05;there was no difference in HSF1 expression between the esophagitis group and the esophageal dysplasia group,P>0.05.HSF1 expression and age,gender,tumor location,tumor size,degree of differentiation,T stage,N stage and M stage are irrelevant,P>0.05,(2)In the ESCC Ⅲ and Ⅳ radiotherapy and chemotherapy group,non-tumor lesions in the HSF1 low expression group were more significantly relieved after radiotherapy and chemotherapy than the HSF1 high expression group,P=0.031:The tumor diameter after radiotherapy and chemotherapy in the HSF1 low expression group was more significantly reduced than that in the HSF1 high expression group,P=0.003;the CEA value of the HSF1 low expression group after radiotherapy and chemotherapy decreased more significantly than the HSF1 high expression group,P<0.001;the HSF1 low expression group is more sensitive to radiotherapy and chemotherapy than the HSF1 high expression group,P=0.005.(3)In the ESCC surgical resection group,the 3-year survival period was related to age,HSF1 expression,T stage and N stage,P<0.05;and it was no related to gender,tumor location,tumor diameter and degree of differentiation,P>0.05;HSF1 low expression group has a higher 3-year overall survival rate than HSF1 high expression group,P=0.016.(4)In ESCC Ⅲ and Ⅳ radiotherapy and chemotherapy group,patients’ 1-year,2-year,and 3-year overall survival rates of the HSF1 low expression group were higher than those of the HSF1 high expression group,P<0.05 and HSF1 low expression group has a higher 3-year overall survival rate than HSF1 high expression group,P=0.03.Conclusion:(1)HSF1 is highly expressed in ESCC and the higher the HSF1 expression,the worse the prognosis of patients.(2)In the ESCC Ⅲ and Ⅳ radiotherapy and chemotherapy group,the higher the expression of HSF1,the worse the sensitivity of patients to radiotherapy and chemotherapy;(3)In the ESCC surgical resection group and the ESCC Ⅲ and Ⅳ radiotherapy and chemotherapy group,the 3-year overall survival rate of the HSF1 low expression group was all higher than that of the HSF1 high expression group. |
