Objective: To improve the awareness of otolaryngology head and neck surgeons on laryngopharyngeal reflux disease(LPRD),the questionnaire survey and analysis of outpatients in Chongqing area were conducted by using reflux symptom index(RSI)scale and reflux fingding score(RFS)scale.To investigate the prevalence and effecting factors of laryngopharyngeal reflux disease in outpatients of Otolaryngology Head and Neck Surgery Department in Chongqing area,and to provide basis and methods for early detection,clinical diagnosis and treatment of the disease.Methods: Multi-center cross-sectional survey method and systematic sampling method were used to select outpatients from Otolaryngology Head and Neck Surgery Department at fifteen hospitals in Chongqing from August to November in 2019.Under the condition of the patients’ informed consent,the RSI scale was investigated,and electronic fiber laryngoscope was used.Then,the RFS scale was rated by a professional surgeon to evaluate the throat symptoms.At the same time,the information of the relevant dietary habits,psychological status and physical status were collected.Then,the above factors were analyzed.Results: A total of 3274 questionnaires were sent out,and 3234 valid questionnaires were finally collected,including 385 positive cases and 2849 negative cases.The prevalence of LRPD in Otolaryngology Head and Neck Surgery Outpatient Department in Chongqing was 11.90%(385/3234).The prevalence rate was 12.63%(173/1378)in males and 11.42%(212/1856)in females,with a statistically significant difference(p<0.05).The prevalence rates of 19 to 39 years old,40 to 59 years old,60 to 79 years old,80 to 99 years old were 11.88%,12.23%,11.58%,6.66%,respectively,and the differences between 40 to 59 years old group and 80 to 99 years old group was statistically significant(p<0.05).The prevalence rates of students,staff,workers,farmers and businessmen were 5.18%,8.43%,19.04%,18.55% and 16.52%,respectively,and the differences among different occupation were statistically significant(p<0.05).Approximately half of the total participants had persistent throat clearing(symptom 2),excessive sputum or runny nose(symptom 3),and foreign body sensation in the pharynx(symptom 8),with the lowest incidence of obstruction in swallowing food,water,or pills(symptom 4).RSI scores were strongly associated with foreign body sensation in the pharynx(symptom 8)and persistent throat clearing(symptom 2),and were moderately associated with all other symptoms.Coughing after eating or lying down(Symptoms 5)and annoying coughs(Symptoms 7)are closely related and often co-exist in LPRD.The positive rate of RFS was 9.43%,and the difference between genders was statistically significant(p<0.05).Approximately two-third of the total population had mucus in the larynx on fiberoptic laryngoscopy.In LPRD-positive patients,the most common signs were mucus attachment in the laryngeal mucosa(sign 8),false vocal folds(sign 1),and erythema or hyperemia(sign 3).In LPRD negative patients,pseudovocal sulcus(sign 1)and granuloma(sign 7)are rarely found.Multivariate Logistic analysis showed that smoking,drinking,spicy diet,overeating,high-fat diet,long sitting after meal,eating before sleep,constipation,anxiety,depression,nasal congestion and runny nose were closely related to LPRD.Conclusion: The prevalence rate of laryngopharyngeal reflux disease in outpatients of otolaryngopharyngology head and neck surgery in Chongqing area is about 11.90%.Gender,age and occupation are closely related to the prevalence rate of LPRD,and the foreign body sensation in the pharynx and persistent throat clearing are the most common in LPRD.Smoking,drinking,spicy diet,overeating,high-fat diet,sitting after meals,eating before bed,constipation,anxiety,depression,nasal congestion and runny nose increase the risk of LPRD,and are risk factors for LPRD.The risk factors associated with LPRD should be corrected.
