| Objective:Many patients who cannot retain teeth due to trauma and other reasons hope that immediate implant technology can restore aesthetic and functional.However,the immediate implant technique cannot prevent the absorption of the labial bone plate after tooth extraction,and it faces aesthetic risks such as gingival recessionand thread exposure in the long term.Even with the use of GBR bone grafting technology,the absorption of labial bone plate cannot be prevented,only the contour can be maintained,and there is a large surgical trauma and postoperative reaction,increasing the economic burden of patients.Dr.Hurlzeler in Germany proposed the dental barrier technology to solve the problem of posterior lip bone resorption of tooth extraction.This technology requires that shield should not be removed during the operation,and 1mm space should be kept between shield and implant to maintain a complete periodontal membrane to maintain blood supply.The shield should be retained for a long time without resorption,and new cementum should be formed between shield and implant.In recent years,socket shield technology as a new technology in the field of immediate implantation,has grown rapidly at home and abroad.But in the clinic,it’s complicated.However,the clinical situation is complicated.During implant implantation,it is sometimes difficult to avoid the contact with the retained dental fragments.Whether this will affect the osseointegration of the implant has not been clearly concluded at present.In this study,a Beagle dog mandibular immediate implant model was established to compare the effects of dental implants on the preservation of alveolar bone under different conditions and to explore the composition of new bone between dental implants.Material and methods:six healthy beagle,in bilateral mandibular premolar(P4)and molar crown(M1)cut,saline flushing cooling,using high speed turbine and emery holing needle preparation in the sprue tooth slice(length of 4 mm,thickness is 1.5 mm,the width of 2mm)and maintain its stability,labial side section is located in the side of the lip on the top of the alveolar bone crest about 1 mm,straight head with planting machine,split drill drill grinding ball in addition to the residual root,palatal side and on the surface of the adjacent teeth residual roots,implanted implant.They were randomly divided into three groups: A,B,and C.Group A kept the shield close to the implants,Group B kept1 mm jump gap between the implant and shield,and Group C did not keep shield as the control group.Finally,there were 9 cases in Group A,9 cases in Group B,and 6 cases in Group C.The implant was placed at the ideal position,the ISQ value was measured,and the initial stability reached > 35 N·cm.After tight suture of the covering screws,penicillin was injected intrascularily to prevent infection.After three months will be executed,experimental animals taken CT observation of dental implant position and slice of retention,cut out the implant and the surrounding alveolar bone,resin embedding,grinding,toluidine blue staining to observe the tooth slice between the implant and the hard tissue ingrowth of composition,comparing single factor analysis of variance by SPSS17.0 statistical software version groups of buccal bone absorption rate,with P < 0.05,statistically significant.Results:Gross and histological observation;At 3 months,4 of the 24 implants fell off due to infection and other factors.Finally,6 implants remained in Group A,6 implants remained in Group B,and 6 implants remained in Group C.Mucosal healing in the planting area was intact,and the grafts were well combined with bone when the flap was cut open.The dental implants in group A were stable,the shields were absorbed,and the labial bone plates were complete.In group B,the labial bone plate was complete,and the shields were clear and complete.There was new bone between the implant and the labial bone plate,and the exploration was hard.In group C,the labial bone plate was seriously absorbed,and the implant threads were clearly visible.Three months after operation,new bone trabeculae and capillaries were formed in the roots of shield and implants in group A and B,active osteoblasts were found in the bone trabeculae,and fibrous connective tissue was observed between shield and implants in the crown.In group C,the labial bone plate was partially absorbed,the height was reduced,and the implant bone combined well.Histological analysis: The final mean height of labial vertical bone resorption in group A,B and C was 1.13±0.59 mm,1.17±0.55 mm and 1.67±0.20 mm.Compared with the mean height of labial vertical bone resorption among all groups,group A,group B and group C are very different(P>0.05).Pindicating that the preservation effect of labial dental film on alveolar bone was better than that of the control group.Group A is very similar to group B(P>0.05).The preservation of labial alveolar bone was promoted by the retention of 1mm jump gap between the dental implant and the dental implant,and the effect was similarConclusion:It was found that the retention of buccal dental plates during immediate implantation did not affect osseointegration,and was beneficial to the preservation of buccal bone plates,but the forward results are uncertain. |
